THE PERFORMING ARTS COLLECTIVE - Liability Waiver & Assumption of Risk
Participation in dance, acrobatics, conditioning, performance, competition, and related physical activities involves inherent risks.
By enrolling my child in The Performing Arts Collective programs, I acknowledge and agree to the following:

Assumption of Risk
I understand participation may involve risks including, but not limited to:
• Falls
• Collisions
• Muscle strains
• Sprains
• Fractures
• Joint injuries
• Acrobatic-related injuries
• Injuries occurring in shared or rented facilities
• Transportation-related risks associated with studio activities
I knowingly and voluntarily assume all risks associated with participation.

Release of Liability
To the fullest extent permitted by law, I release and hold harmless The Performing Arts Collective, its owners, directors, instructors, staff, volunteers, contractors, and any facility used for classes, rehearsals, performances, or events from any claims, demands, damages, injuries, losses, costs, or causes of action arising from participation in studio activities, except in cases of gross negligence or willful misconduct.
Physical Contact Acknowledgment
I understand dance instruction may involve appropriate physical contact for:
• Alignment correction
• Technical instruction
• Skill assistance
• Injury prevention
• Acrobatic spotting and support
I acknowledge such contact is a standard and necessary part of dance education and training.
Indemnification
I agree to indemnify and hold harmless The Performing Arts Collective from claims arising from my student's actions, behavior, or participation in studio activities.
Agreement
By selecting "I Agree," I acknowledge that I have read, understand, and voluntarily accept the risks associated with participation at The Performing Arts Collective.

THE PERFORMING ARTS COLLECTIVE - Medical Authorization & Media Consent

Emergency Medical Authorization
I authorize The Performing Arts Collective, its directors, instructors, employees, and representatives to obtain emergency medical treatment for my child if illness or injury occurs during classes, rehearsals, performances, competitions, camps, workshops, or studio-related activities.
I understand every reasonable effort will be made to contact me prior to treatment. If I cannot be reached, I authorize licensed medical professionals to provide treatment deemed necessary.
I understand that I am solely responsible for any medical expenses incurred.

Health & Medical Information
I agree to inform the studio of any medical conditions, injuries, allergies, restrictions, or concerns that may affect my child's participation.
I understand that failure to disclose relevant medical information may increase the risk of injury.

Media Release
I grant permission for The Performing Arts Collective to photograph, videotape, livestream, or otherwise record my child during classes, rehearsals, performances, competitions, events, and studio activities.
I understand these materials may be used for:
• Studio marketing and advertising
• Social media platforms
• Studio website
• Printed promotional materials
• Recital programs
• Press releases and community publications
• Educational and archival purposes
I understand that all photographs, videos, recordings, and media created by The Performing Arts Collective remain the property of The Performing Arts Collective and may be used without compensation.
Media Consent Selection

By selecting "I Agree," I acknowledge that I have read and understand this Medical Authorization & Media Consent Agreement.
