# Ignite Dance Studio Registration Agreement, Waiver & Release

Thank you for choosing Ignite Dance Studio. Our goal is to provide a positive, safe, and encouraging environment where every dancer can grow in both skill and confidence. By completing registration, you acknowledge that you have read, understood, and agree to the following terms and conditions.

## Assumption of Risk & Release of Liability

I understand that participation in dance classes, rehearsals, performances, conditioning, acro, camps, workshops, and related activities involves physical activity and carries inherent risks of injury, including but not limited to sprains, strains, fractures, or other injuries that may occur despite reasonable precautions.

I voluntarily assume all risks associated with participation at Ignite Dance Studio for myself or my child.

I certify that my child (or I, if registering as an adult participant) is physically able to participate in dance activities. I agree to inform Ignite Dance Studio of any injuries, medical conditions, allergies, or physical limitations that could affect safe participation.

To the fullest extent permitted by law, I release and hold harmless Ignite Dance Studio, its owner, instructors, employees, volunteers, contractors, and representatives from claims, demands, damages, or liability arising from participation, except where caused by gross negligence or willful misconduct.

## Emergency Medical Authorization

If a medical emergency occurs and I cannot be reached, I authorize Ignite Dance Studio to seek emergency medical treatment for my child.

I understand that every reasonable effort will be made to contact a parent or emergency contact as quickly as possible.

I accept full responsibility for any medical expenses resulting from emergency treatment.

## Photo & Video Release

I grant Ignite Dance Studio permission to photograph and record my child during classes, rehearsals, performances, competitions, workshops, camps, and studio events.

I understand these images and videos may be used on the studio website, social media platforms, printed materials, advertisements, promotional videos, and other marketing materials without compensation.

If I do not wish for my child to be included in promotional media, I understand that I must notify the studio in writing.

## Tuition & Payment Agreement

I understand that registration reserves my dancer's place in class and creates a financial commitment for the season.

I agree to pay all tuition, registration fees, competition fees, costume fees, workshop fees, and any other applicable charges according to Ignite Dance Studio's payment schedule.

Tuition is due on the dates established by the studio. Late payments may be subject to applicable late fees.

Returned payments or declined transactions may result in additional processing fees.

## Withdrawal Policy

If I choose to withdraw my dancer, I understand that written notice must be submitted according to the policies outlined in the Parent Handbook.

I understand that failure to attend class does not automatically withdraw my dancer or eliminate financial responsibility for tuition or other charges.

## Attendance

Regular attendance is essential for dancer growth and participation in performances.

I understand that excessive absences may affect choreography placement, recital participation, or competition eligibility at the discretion of the studio.

## Dress Code

I agree to follow Ignite Dance Studio's dress code for all classes.

Appropriate dancewear and footwear help maintain a safe learning environment and allow instructors to provide proper technical corrections.

## Parent & Student Conduct

I agree to contribute to a respectful and positive studio environment.

Parents, dancers, and guests are expected to treat instructors, staff, volunteers, fellow dancers, and other families with kindness and respect.

Harassment, bullying, discrimination, disruptive behavior, or inappropriate conduct may result in disciplinary action or dismissal from the studio.

## Health & Wellness

I agree not to send my child to class if they are experiencing symptoms of a contagious illness or any condition that could place others at risk.

I understand that participation while ill may not be permitted.

## Personal Property

Ignite Dance Studio is not responsible for lost, stolen, or damaged personal belongings, including clothing, dance shoes, electronics, jewelry, or other valuables brought to the studio.

## Studio Policies

I understand that studio policies may be updated as necessary throughout the season. Any changes will be communicated to families through official studio communication channels.

## Acknowledgment

By completing registration, I acknowledge that I have carefully read this Registration Agreement, Waiver, and Release. I understand its contents, voluntarily agree to its terms, and certify that I am at least 18 years of age or am the legal parent or guardian of the participating dancer.

I understand that this agreement is intended to remain in effect for all participation in Ignite Dance Studio programs, classes, rehearsals, performances, workshops, camps, competitions, and related activities unless otherwise updated by the studio.

By signing this agreement, I acknowledge that participation in dance, acrobatics, fitness, conditioning, performances, workshops, and all related activities offered by Ignite Dance Studio involves inherent risks, including but not limited to falls, collisions, sprains, strains, fractures, and other injuries.

I understand that participation is voluntary and that I (or my child) am physically able to participate in these activities. I agree to notify Ignite Dance Studio of any medical conditions, injuries, allergies, or limitations that may affect participation.

I understand that while Ignite Dance Studio takes reasonable precautions to provide a safe environment, accidents may occur. On behalf of myself, my child, and our heirs, executors, and representatives, I voluntarily assume all risks associated with participation and release, waive, and discharge Ignite Dance Studio, its owner, instructors, employees, independent contractors, volunteers, and the owners of any facilities used for classes or events from any and all claims, demands, liabilities, damages, costs, or causes of action arising out of or related to participation, except in cases of gross negligence or willful misconduct where prohibited by law.

In the event of an emergency, I authorize Ignite Dance Studio to obtain emergency medical treatment for myself or my child if I cannot be reached immediately. I understand that I am solely responsible for any medical expenses incurred.

I grant Ignite Dance Studio permission to photograph and/or record myself or my child during classes, rehearsals, performances, workshops, events, and promotional activities. I understand that these images and videos may be used for marketing, advertising, social media, the studio website, and other promotional materials without compensation.

I acknowledge that I have read and understand this waiver, have had the opportunity to ask questions, and voluntarily agree to its terms.

Ignite Dance Studio Payment Policy

By registering for classes at Ignite Dance Studio, you agree to the following payment policies:

* Tuition is based on the full dance season (September through June) and divided into equal monthly installments for your convenience. Monthly tuition remains the same regardless of the number of classes held in a particular month.

* Tuition is due on the **1st of each month**.

* A **$15 late fee** will be applied to accounts with unpaid balances after the **10th of the month**.

* Tuition is non-refundable and non-transferable.

* Missed classes do not qualify for refunds, credits, or prorated tuition. Makeup classes may be offered when appropriate and at the discretion of Ignite Dance Studio.

* Accounts with outstanding balances may result in a dancer being unable to participate in classes, performances, competitions, costumes, or other studio activities until the account is brought current.

* Any returned payments or declined transactions may be subject to additional processing fees.

* Families who choose to withdraw from classes must submit written notice by email no later than the **25th of the month** in order to discontinue tuition for the following month. Tuition is the responsibility of the registered family until written notice is received.

* Costume fees, competition fees, registration fees, recital fees, merchandise, and any other studio charges are separate from monthly tuition unless otherwise stated.
