I understand that participation in dance classes, rehearsals, performances, camps, special events, and other activities offered by OnPointe Dance Studio involves physical activity and carries an inherent risk of injury. I voluntarily allow my child (or myself, if an adult participant) to participate in these activities.

I release and hold harmless OnPointe Dance Studio, its owners, instructors, staff, volunteers, and affiliates from any claims, injuries, damages, losses, or liabilities arising from participation in studio activities, except in cases of gross negligence or willful misconduct.

I certify that I am physically capable of participating, or allowing my child to participate, in studio activities and have disclosed any medical conditions, injuries, allergies, or limitations that may affect participation.

In the event of an emergency, I authorize OnPointe Dance Studio staff to seek emergency medical treatment for myself or my child if I cannot be reached immediately. I understand that any medical expenses incurred are my responsibility.

I acknowledge that I have read and agree to follow all studio policies, procedures, rules, and expectations established by OnPointe Dance Studio. I understand that studio policies may be updated periodically and that continued participation constitutes acceptance of those policies.

I understand that photographs and videos may be taken during classes, rehearsals, performances, camps, and studio events for promotional, marketing, social media, website, and studio-related purposes. I grant permission for OnPointe Dance Studio to use such images and recordings unless I provide written notice requesting otherwise.

By signing below, I acknowledge that I have read, understood, and agree to the terms of this Liability Waiver and Studio Agreement.
