I have thoroughly reviewed all studio policies and agree to abide by all guidelines presented to me.

I understand that participation in dance classes, rehearsals, performances, workshops, and related activities involves physical activity and carries inherent risks of injury. I voluntarily choose to participate and assume all risks associated with my participation.

I release, waive, and discharge the dance studio, its owners, instructors, employees, volunteers, and agents from any and all claims, liabilities, demands, or causes of action arising out of or related to any injury, illness, loss, or damage that may occur as a result of participation in studio activities, except where prohibited by law.

I certify that I (or my child) am physically capable of participating in dance activities and agree to follow all studio rules and safety instructions. In the event of a medical emergency, I authorize the studio to obtain emergency medical treatment if necessary and understand that I am responsible for any resulting medical expenses.

Parent/Guardian Acknowledgment (for minors):

As the parent or legal guardian of the participant, I have read and understand this waiver and consent to my child’s participation in all studio activities under the terms stated above.

I hereby grant permission to the dance studio, its representatives, employees, and authorized photographers/videographers to photograph, record, and use images, video recordings, audio recordings, and likenesses of myself (or my child) during classes, rehearsals, performances, competitions, events, and other studio-related activities.

I understand and agree that these photographs, videos, and recordings may be used by the studio for promotional, educational, advertising, marketing, social media, website, print, and other lawful business purposes without further notice, approval, or compensation.

I release and discharge the studio and its representatives from any claims arising from the use, publication, reproduction, or distribution of such photographs, videos, or recordings.

I understand that the studio may edit, modify, or combine such media with other content and that I waive any right to inspect or approve the finished product.

This authorization shall remain in effect unless revoked in writing, except for materials already published or distributed prior to such revocation.

I acknowledge that I have received, read, and agree to abide by The Crosby Dance Collective Studio Handbook. I understand it is my responsibility to remain familiar with studio policies throughout the season.
