In the event of a major or minor medical emergency, I authorize Kathryn Kingery-Benvegnu, Kate Benvegnu, Dayton Academy of Dance, LLC/KDP employees, and/or appointed representatives to seek medical care and give medical consent for my student(s) if I cannot be reached.

By enrolling myself and/or my student(s), I certify that I/my student(s) are in good health, physically able to participate, and have no known physical condition that would be negatively affected by participation in dance classes or related activities.

I understand that participation in dance and related activities involves inherent risks, including the risk of injury.

I release Dayton Academy of Dance, LLC/KDP, its owners, directors, employees, representatives, heirs, and assigns from any and all responsibility, claims, or liability arising from participation in classes, rehearsals, recitals, travel, or any other activities conducted by Dayton Academy of Dance, LLC/KDP, whether on its premises or any other premises.

By enrolling myself and/or my student(s), I acknowledge and agree that Dayton Academy of Dance, LLC/KDP is not responsible for the personal belongings of students, parents, visitors, spectators, or friends while on its premises or any other premises used for classes, rehearsals, recitals, travel, or related activities.

I give Dayton Academy of Dance, LLC permission to use my student's image for marketing and promotional purposes.
