*Tuition is billed monthly
*There will be no credits and/or refunds for classes missed due to vacation or illness. Please arrange with the front desk for a make-up class as soon as possible. Make-up class must be taken within 1 month of absence.
*We do not issue refunds on tuition. Returned checks will be subject to a $35 fee.
*Students are not permitted in class if their accounts are more than 2 months past due.
*If a student withdraws from a class, the office MUST be notified by email or phone. Otherwise, the tuition for the reminder of the year is still due and you will continue to have tuition charged.
PARTICIPANT AGREEMENT, ASSUMPTION OF RISK & RELEASE
By registering for classes, programs, rehearsals, performances, workshops, camps, auditions, or other activities offered by The Redding Arts Project ("RAP"), I acknowledge and agree to the following:
ASSUMPTION OF RISK
I understand that participation in dance, theatre, tumbling/acrobatics, and other performing arts activities involves inherent risks of injury. These risks may include, but are not limited to, falls, slips, collisions, sprains, strains, fractures, dislocations, overuse injuries, injuries resulting from jumps, turns, lifts, partnering, tumbling or other movements, and injuries associated with stages, sets, props, equipment, rehearsals, performances, or activities at off-site venues.
I understand that injuries may occur even when reasonable safety precautions are taken and activities are properly supervised.
I voluntarily choose to participate, or voluntarily permit the minor participant for whom I am responsible to participate, with knowledge of these risks.
PARTICIPANT RESPONSIBILITY
I understand that participation is voluntary. I agree to follow reasonable instructions and safety rules established by RAP and its instructors.
I agree to inform RAP of any known injury, physical limitation, medical restriction, or other circumstance that may affect the participant's ability to safely participate.
I understand that RAP instructors and staff may modify, restrict, or discontinue a participant's participation when they reasonably believe it is necessary for safety.
RELEASE OF LIABILITY
In consideration of being permitted to participate in RAP activities, I, on behalf of myself and, to the extent permitted by law, on behalf of the minor participant for whom I am responsible, agree to release and hold harmless The Redding Arts Project, its owners, officers, directors, employees, instructors, independent contractors, volunteers, agents, representatives, and affiliated organizations and individuals involved in providing RAP activities (collectively, the "Released Parties") from claims arising out of or related to participation in RAP activities, including claims for personal injury, illness, property damage, or death, to the extent permitted by California law.
This release is intended to apply to risks inherent in the activities and to claims arising from the ordinary negligence of a Released Party, to the extent such a release is permitted and enforceable under applicable law.
This agreement does not release claims that cannot legally be released under California law.
EMERGENCY MEDICAL CARE
In the event of an emergency and if I cannot be reached, I authorize RAP and its designated representatives to obtain reasonable emergency medical assistance for the participant when, in their reasonable judgment, such assistance is necessary.
I understand that I am responsible for any resulting medical, transportation, or treatment expenses.
PERSONAL PROPERTY
I understand that RAP is not responsible for lost, stolen, or damaged personal belongings brought to classes, rehearsals, performances, or other activities.
PARTICIPANT CONDUCT
I agree that I, or the minor participant for whom I am responsible, will:
Follow reasonable instructions from RAP instructors and staff;
Treat instructors, staff, volunteers, fellow participants, and members of the public respectfully;
Follow studio, rehearsal, performance, and venue safety rules;
Use equipment appropriately; and
Report injuries, unsafe conditions, or safety concerns to an instructor or RAP staff member.
RAP reserves the right to remove a participant from an activity when conduct presents a safety concern or substantially interferes with the learning environment.
ACKNOWLEDGMENT & ELECTRONIC SIGNATURE
By checking the box below and electronically signing this agreement, I acknowledge that:
I have read and understand this Participant Agreement, Assumption of Risk & Release;
I understand the risks associated with participation in RAP activities;
I voluntarily accept those risks;
I agree to the terms of this agreement;
If registering a minor, I represent that I am the minor's parent or legal guardian and have authority to provide the permissions and consents contained in this agreement; and
I understand that I may seek independent legal advice before agreeing to this document.
