I understand that tuition is due at the time of enrollment, or before the first class. Tuition for Fall and Spring sessions will be processed through automatic payment on file to ensure timely and seamless enrollment. If an alternate payment arrangement is needed, I agree to contact the studio directly to request approval for a different payment plan. If a monthly payment plan is approved, payment is due by the first day of each month. A $20 late fee will be applied on the 15th day of the month if payment has not been received. If my child withdraws from class, I agree to provide written notice to the studio; otherwise, tuition charges will continue to accrue. All returned payments and checks are subject to a $25 fee.

Studio MDC values the privacy of our dancers and families. Information collected through registration is used only for studio operations and communication purposes.

By enrolling, I grant Studio MDC permission to use photographs and/or videos of my dancer taken during classes, rehearsals, performances, competitions, and events for promotional and marketing purposes, including social media and website content. If I wish to opt out, I understand I must notify the studio in writing.
