I/We the parent/guardian do hereby permit our child to participate in Cheerleading, Tumbling, Stunting, Dance, Acrobatics and or other physical activity while an athlete at Empire Xtreme. By granting permission to participate in this program I/We herby assume full responsibility for our child”s personal safety and release Empire Xtreme, its supervisors, staff, instructors, whether paid or volunteer from any and all liabilities that may occur from injury including but not limited to death. I/we understand that there is personal risk involved in any activity that includes motion, height, and waive any claim for compensation for accidental injury, disability, and or death while at Empire Xtreme or while participating in an Empire Xtreme activity away from the gym. I/We furthermore herby agree to hold harmless Empire Xtreme ,supervisors,staff, instructors, whether paid or volunteer, against any and all claims which may arise while participating at Empire Xtreme. I/We have received and read the Empire extreme rules and Regulations and agree that our child will follow all the rules pertaining to the gym, other athletes, staff, coaches, and instructors.
