In case of physical injury or medical emergency, I authorize the Studio to make necessary arrangements to
transport myself or my child or participant for whom I am guardian to a medical treatment facility as
necessary. All such transportation and medical treatment will be at my sole cost and expense.
In signing this Release, I acknowledge and represent that I have fully informed myself of the content of
the waiver. I agree that I have read the agreement before I signed it and I understand that I sign this
document of my own free will and that no oral representations, statements, or inducements, apart from
the written statements contained herein, have been made. I hereby confirm that I am at least 18 years
of age and of sound mind. I hereby execute this release for full and complete consideration fully
intending to be bound by the same. I further state that there are no health-related reasons or problems
which preclude or restrict my or my child’s or the person over whom I am guardian’s participation in this
activity. I further agree that I have been provided sufficient time to review this Agreement and that I
have either sought legal advice or have waived my right to do so

Parent Handbook Agreement

By registering a dancer with Turtleford Dance Studio Ltd., the parent(s) and/or legal guardian(s) acknowledge that they have read, understood, and agree to abide by all policies, procedures, and expectations outlined in the Parent Handbook.

Registration constitutes acceptance of all studio policies, including but not limited to:

* Tuition and payment policies
* Late payment policies and late fees
* Costume and competition fee policies
* Attendance policies
* Class prerequisites
* Competition expectations
* Dress code requirements
* Studio code of conduct
* Communication policies
* Media and photography policies
* Health and safety policies
* Withdrawal and refund policies

Parents/guardians understand that it is their responsibility to remain familiar with the contents of this handbook and any policy updates communicated by Turtleford Dance Studio Ltd. throughout the season.

Failure to read the handbook does not exempt families from the policies contained within it.

Payment Policy

To keep the studio operating smoothly and ensure all commitments can be met throughout the season, all accounts must remain current.

* Monthly tuition for regular season enrolment is due on the 1st of each month from September through April.
* Costume fees are due October 1st.
* Competition fees are due November 1st.
* Solos, duets, trios, private lessons, workshops, and session programs are invoiced separately and are due by the invoice due date.
* All fees are subject to applicable taxes.

Late Payments

Payments not received by the invoice due date will incur a $10.00 late fee per day until the outstanding balance has been paid in full.

Late fees apply to all outstanding invoices, including but not limited to:

* Monthly tuition
* Costume fees
* Competition fees
* Solos
* Duets
* Trios
* Private lessons
* Session programs
* Any other invoice issued by Turtleford Dance Studio Ltd.

Late fees will not be waived for any reason.

Accounts that remain unpaid may result in dancers being unable to participate in classes, competitions, performances, or other studio activities until the account has been brought into good standing.

I understand and agree that it is my sole responsibility to safeguard my personal property while attending
or participating in any classes, rehearsals, workshops, or performances. I hereby release the Indemnified
Parties from all liability for loss or damage to my personal property while attending or participating in
classes, rehearsals, workshops, or performances. I also agree to abide by any rules, regulations and policies
set forth by the Studio as are set and or amended from time to time.

The Studio has the right to use photographs and videos taken during or at any activities associated with
the Studio and may use such photos or videos as promotional or instructional materials at the discretion
of the Studio. Anyone who wishes to not be included in such photos or videos must notify the Studio in
advance of participating in any event or activity.

TURTLEFORD DANCE STUDIO LTD.

WAIVER AND RELEASE OF LIABILITY FORM

All participants MUST complete this form

Any and all participants and students (or their guardians) must complete this form before participating in
any classes at or in any way connected with Turtleford Dance Studio Ltd. (the “Studio”). If the participant
is under age 18, a parent and/or guardian must also sign this form. No participant will be granted access
or be allowed to participate in any rehearsal, practice, or competition without have first having signed this
form properly.
LIABILITY WAIVER
I,___________________________________, recognize and understand the risks of physical injury
inherent to dance and all associated activities and I fully assume those risks. I hereby release the Studio
as well as its employees, dance teachers, contractors, shareholders, and directors, as well as their assigns
and successors (the “Indemnifies Parties”), from all any and all liability including for injuries sustained
(including death) or illnesses contracted while attending or participating in any activities related to the
Studio, including dance classes, rehearsals, workshops, or performances. I agree to indemnify, defend, and
hold harmless the Indemnified Parties for liabilities, costs and judgments arising from acts of omissions
committed by me or my child which result in injury or damage to any person or property.

In signing this Release, I acknowledge and represent that I have fully informed myself of the content of
the waiver. I agree that I have read the agreement before I signed it and I understand that I sign this
document of my own free will and that no oral representations, statements, or inducements, apart from
