All That Jazz Dance Company Waiver and Release of Liability

Liability Release
I, the undersigned, acknowledge that participation in dance and gymnastics activities involves inherent risks. I hereby release and hold harmless All That Jazz Dance Company, its owners, instructors, staff, and affiliates from any and all liability for injuries, damages, or losses that may occur as a result of participation in classes, events, or any related activities.

I understand that it is my responsibility to ensure that my child is physically fit and capable of participating in such activities, and I accept full responsibility for any injuries sustained during participation.

Refund Policy
All payments made to All That Jazz Dance Company are final and non-refundable. Monthly tuition payments are structured to account for scheduled closures due to holidays, and no refunds will be issued for any missed classes.

Photography Release
I grant All That Jazz Dance Company permission to photograph and/or video my child during classes, events, and performances. I understand that these images may be used for promotional purposes, including but not limited to social media, website content, and printed materials. If I do not wish for my child’s images to be used, I will provide written notice to All That Jazz Dance Company.

I acknowledge that I have read and understood this waiver and release of liability.
