I understand that dance involves physical activity and carries inherent risks. I voluntarily assume all risks associated with participation in classes, rehearsals, performances, and studio‑related activities.

I release Kreme De La Kreme Dance Studio, its staff, and affiliates from liability for injuries or damages that may occur.

In the event of an emergency, I authorize staff to seek appropriate medical care for my dancer. I understand that medical expenses are my responsibility. Type Below:

Purpose of Release
Kreme De La Kreme Dance Studio uses photos and videos to highlight student progress, celebrate achievements, and promote studio events. These images may appear on our official social media platforms, website, printed materials, or other studio‑related publications.

By signing below, I grant Kreme De La Kreme Dance Studio permission to photograph, record, and use images or videos of my dancer for promotional and educational purposes. I understand that:

Images may be used on platforms such as Instagram, Facebook, TikTok, the studio website, and printed marketing materials. My dancer’s first name may be used, but last names will not be shared without additional permission. All media will be used respectfully and in alignment with the studio’s mission and values. I will not receive compensation for the use of these images or recordings. I may revoke this consent at any time by submitting a written request to the studio office. Opt‑Out Option

Enrollment & Tuition

Tuition is non‑refundable and must be paid on schedule. Missed classes are non‑refundable; makeup classes may be offered when available.

Attendance & Conduct

Dancers must arrive on time and follow studio etiquette.Disruptive behavior may result in dismissal from class.
Class Placement

Placement is determined by instructors to ensure safety and proper progression.

Studio Policies

No food or gum in studios. Label personal items; The studio is not responsible for lost belongings.
