As the legal guardian of the aforementioned minor athlete, I authorize Empire Cheer Inc. and its representatives to obtain and consent to medical treatment for my child if I cannot be reached. I also grant permission for Empire Cheer Inc. and its representatives to administer any necessary emergency care to stabilize or improve any injury or condition sustained during activities related to Empire Cheer Inc., including instruction, practices, or performances. This authorization applies without the need for a prior determination of a life-threatening emergency or risk of permanent injury.

I acknowledge that participation in activities involving motion, height, and athletic performance carries an inherent risk of serious injury. I agree to hold harmless Empire Cheer Inc., including its staff, officers, and representatives, from any liability for injuries or resulting expenses.

I hereby release and discharge any and all claims against Empire Cheer Inc. and its affiliates. While Empire Cheer Inc. is committed to maintaining high safety standards and procedures, it cannot assume responsibility for accidents, injuries, or illnesses that may occur. I understand and accept this risk.

I also acknowledge that Empire Cheer Inc. strongly recommends that all athletes maintain personal health insurance coverage to assist with potential medical costs.

Additionally, I authorize Empire Cheer Inc. to use photographs, videos, and/or other likenesses of my child for promotional or marketing purposes, and I waive any rights to compensation or ownership related to such materials.
