1. Enrollment Agreement
By signing this waiver, I understand that enrollment in the dance program is a commitment for the duration of the agreed session, season, or term. I agree to follow all studio policies, schedules, and guidelines.
2. Tuition & Payment Policy
I acknowledge and agree that:
Monthly tuition is due on or before the agreed-upon due date each month.
Tuition remains the same regardless of attendance, missed classes, or holidays unless otherwise stated.
Late payments may be subject to additional fees.
Failure to make payments may result in suspension or removal from classes.
3. Additional Fees & Expenses
I understand and agree that participation in the dance program may require additional costs, including but not limited to:
Costumes
Recital/performance fees
Competition fees (if applicable)
I agree to pay all required fees by their specified due dates once they are communicated and accepted.
4. Refund Policy
I understand that:
All tuition and fees paid are non-refundable unless otherwise stated by the studio.
Costumes and recital fees are non-refundable once ordered or processed.
5. Withdrawal Policy
I agree to provide written notice prior to withdrawing from the program. I understand I am responsible for all payments due during that notice period.
6. Liability Waiver
I acknowledge that dance involves physical activity and carries a risk of injury. I voluntarily assume all risks associated with participation. I release and hold harmless the studio, instructors, and staff from any liability for injuries, accidents, or damages that may occur during classes, rehearsals, performances, or related activities.
7. Agreement & Acknowledgment
By signing below, I confirm that I have read, understood, and agree to all terms outlined in this waiver. I accept full financial responsibility for all tuition, fees, and additional costs associated with participation.
