I have read this waiver in full and understand its terms. I acknowledge that I am giving up certain legal rights, including the right to sue.

I voluntarily agree to all terms and conditions stated above.

I, as the legal parent/guardian of the above-named participant(s), understand that participation in gymnastics, tumbling, trampoline, cheer, tricking, dance, and all related physical activities involves inherent risks. These risks include, but are not limited to, falls, collisions, improper landings, equipment failure, and physical exertion which may result in serious injury including permanent disability, paralysis, or death.
I knowingly and voluntarily assume all risks associated with participation in any and all BIO Gymnastics programs and activities.

1. ACKNOWLEDGMENT OF GENERAL WAIVER
I acknowledge that I have previously signed the BIO Gymnastics General Liability Waiver. This Camp Waiver serves as an addendum and applies specifically to participation in camp programs, including extended hours and additional activities.

2. ASSUMPTION OF ADDITIONAL CAMP RISKS
I understand that camp participation may include longer activity periods, varied programming, group play, games, obstacle courses, and other physical activities.
I acknowledge that these activities may involve increased risk due to fatigue, group interaction, and extended participation time.

3. DROP-OFF & PICK-UP PROCEDURES
I understand and agree to the following:
Camp is a drop-off program only
I must check my child in and out daily
Each family will be assigned a unique pick-up PIN code, which will be sent via email
The assigned PIN code must be provided at pick-up in order for a child to be released

BIO Gymnastics will not release a child to any individual without the correct PIN code, regardless of authorization status
It is my responsibility to share the PIN code only with authorized pick-up individuals

A valid photo ID may be required in addition to the PIN code
Late pick-up fees may apply

4. ATTENDANCE, CANCELLATIONS & REFUNDS
Camp fees are non-refundable and non-transferable once the camp week has started
Cancellations made 14 or more days prior to the registered camp week will receive a full refund
Cancellations made 8–13 days prior to the registered camp week will receive a partial refund
Cancellations made 7 days or less prior to the registered camp week are not eligible for a refund
No refunds or credits will be issued for missed camp days
Make-up days are not offered for camp programs

5. EXTENDED CARE & LATE PICK-UP POLICY
If my child is enrolled in early drop-off or aftercare:
I understand additional fees apply
My child will be supervised only during scheduled camp hours and registered extended care times
I acknowledge that:
A 10-minute grace period is allowed for pick-up
After the grace period, I will be charged the applicable before care or aftercare rate

6. FOOD, SNACKS & ALLERGIES
I understand that:
I am responsible for providing snacks and/or lunch unless otherwise specified
BIO Gymnastics is not responsible for managing food allergies unless clearly disclosed
I must notify staff of any allergies or dietary restrictions

7. FIELD TRIPS
I understand that camp may include off-site field trips as part of the weekly schedule.
I acknowledge that:
Field trips involve additional supervision and transportation outside of the gym
I will be informed in advance of any scheduled field trips
I will have the option to allow my child to participate or opt out of specific field trips

8. MEDICAL AUTHORIZATION (CAMP-SPECIFIC)
In addition to the general waiver, I authorize BIO Gymnastics staff to:
Administer basic first aid
Apply sunscreen (if provided)
Seek emergency medical care if necessary
I understand that every effort will be made to contact me in the event of an emergency.
I accept full responsibility for any medical expenses incurred.

9. BEHAVIOR & PARTICIPATION EXPECTATIONS
For the safety and enjoyment of all participants:
Children must follow instructions and safety rules at all times
Bullying, unsafe behavior, or repeated disruption may result in removal from camp
No refunds will be given if a child is dismissed due to behavior

10. PERSONAL BELONGINGS
I understand that:
BIO Gymnastics is not responsible for lost, stolen, or damaged personal items
All items brought to camp should be clearly labeled with my child’s name
I should avoid sending valuables with my child

11. ACKNOWLEDGMENT OF UNDERSTANDING
I have read and understand all camp policies and agree to all terms outlined above. I acknowledge that I am giving up certain legal rights, including the right to sue.

I understand that BIO Gymnastics operates as a drop-off facility for classes and certain programs. I acknowledge that:
My child will be under supervision only during their scheduled class or program time
BIO Gymnastics is not responsible for children before or after their scheduled time

I must ensure timely drop-off and pick-up
I agree to follow all pick-up authorization procedures.

1. ACKNOWLEDGMENT OF GENERAL WAIVER

2. PERMISSION TO PARTICIPATE
I give permission for my child to participate in selecrred field trip organized by BIO Gymnastics.
I understand that this trip will take place outside of the BIO Gymnastics facility and may include travel and activities not directly controlled by BIO Gymnastics.

3. ASSUMPTION OF RISK
I understand that participation in a field trip includes additional risks, including but not limited to:
Transportation to and from the location
Public environments and shared spaces
Interaction with individuals outside of BIO Gymnastics
Activities specific to the field trip location
I voluntarily assume all risks associated with my child’s participation.

4. TRANSPORTATION AUTHORIZATION
I understand and agree that:
Transportation may include walking, staff-supervised transport, or third-party transportation services
My child will be supervised by BIO Gymnastics staff during transportation and at the destination
BIO Gymnastics is not responsible for conditions, incidents, or actions that occur at third-party locations

5. SUPERVISION & EXPECTATIONS
I understand that:
Children must follow all staff instructions and safety rules at all times
Failure to follow directions may result in removal from activities or restricted participation
BIO Gymnastics staff will provide supervision, but cannot control all external environments

6. MEDICAL AUTHORIZATION
I authorize BIO Gymnastics staff to:
Administer basic first aid if needed
Seek emergency medical care if necessary
I understand that every effort will be made to contact me in the event of an emergency.
I accept full financial responsibility for any medical treatment provided.

7. FOOD, ALLERGIES & PERSONAL NEEDS
I understand that:
I am responsible for providing any necessary food, snacks, or special items unless otherwise specified
I must clearly communicate any allergies, medical conditions, or special needs

8. PERSONAL BELONGINGS
I understand that:
BIO Gymnastics is not responsible for lost, stolen, or damaged items
All belongings should be clearly labeled with my child’s name
Valuables should not be brought on the trip

9. LIABILITY RELEASE
I agree to release and hold harmless BIO Gymnastics, its owners, staff, and affiliates from any and all liability for injuries, damages, or losses that may occur during the field trip, including transportation and off-site activities.

10. ACKNOWLEDGMENT OF UNDERSTANDING
I have read and understand all field trip policies and agree to all terms outlined above. I acknowledge that I am giving up certain legal rights, including the right to sue.

I agree to keep my child home if they are sick or showing symptoms of illness, including fever, vomiting, or contagious conditions.
I understand that BIO Gymnastics reserves the right to deny participation if a child appears ill.

I agree to indemnify and hold harmless BIO Gymnastics and all associated parties from any loss, liability, damage, or costs, including attorney fees, that may occur due to my child’s participation in any program.

1. TUITION & ENROLLMENT
Tuition is a monthly fee that reserves my child’s spot in class, not based on attendance
Enrollment will continue automatically each month until proper cancellation is submitted
Tuition rates are subject to change with notice

2. PAYMENT METHODS
I understand and agree that:
BIO Gymnastics offers the option of auto-pay or manual payment
Payments may be made by card, cash, or check
If enrolled in auto-pay, tuition will be processed automatically using the payment method on file
If not enrolled in auto-pay, it is my responsibility to ensure payment is made on time

3. PAYMENT DUE DATES & LATE POLICY
Tuition is due on the 1st of each month
A late fee will be applied after the 8th if payment has not been received
If tuition is not paid by the 20th, my child’s spot in class will not be held
BIO Gymnastics reserves the right to remove enrollment due to non-payment

4. NO REFUNDS POLICY
All payments are non-refundable and non-transferable
No refunds or credits will be issued for missed classes, holidays, illness, or schedule conflicts
Enrollment secures a spot, regardless of attendance

5. MAKE-UP POLICY
Participants may receive make-up tokens for missed classes
Make-up tokens are limited to 2 per month, per athlete
All make-up tokens must be used within 3 months of the original absence
Make-up classes are subject to availability and are not guaranteed
Expired or unused tokens will not be extended, credited, or refunded

6. WITHDRAWAL & CANCELLATION POLICY
A 30-day written notice via email is required to cancel enrollment
Verbal notice, text messages, or non-attendance do not qualify as cancellation
Tuition will continue to be billed during the 30-day notice period
Failure to provide proper notice will result in continued billing responsibility

7. FAILED PAYMENTS
If a payment is declined, I understand that I am responsible for submitting payment immediately
Additional fees may apply for declined payments
Continued non-payment may result in removal from the program

8. CLASS HOLDS & ACCOUNT STATUS
BIO Gymnastics does not offer refunds or credits for temporary absences
Any hold or pause options (if offered) must be approved in advance and may be subject to specific terms

9. AGREEMENT & ACKNOWLEDGMENT
I have read and understand all payment and billing policies. I agree to all terms outlined above and understand that these policies are in place to ensure consistent class availability and program quality.

BIO Gymnastics regularly takes photos and videos during classes, events, and activities for use in marketing, social media, website content, and promotional materials.

By checking this box, I acknowledge and agree that my child may be photographed and/or recorded and that these images may be used by BIO Gymnastics without compensation.

I understand that if I wish to opt out of photo and video use, I must update my preference by selecting the opt-out option within my account settings.

I acknowledge that it is my responsibility to ensure my account preferences are accurate and up to date.

I acknowledge and agree to the following policies set by BIO Gymnastics:

Tuition & Payments
Tuition is ongoing and reserves my child’s spot in class
All payments are non-refundable and non-transferable
No credits or refunds will be issued for missed classes

Make-Up Policy
If my child misses a class, they may receive a make-up token
Make-up tokens are limited to 2 per month, per athlete
All make-up tokens must be used within 3 months of the original absence
Make-up classes are subject to availability and are not guaranteed
Unused or expired tokens will not be extended, credited, or refunded

Attendance & Missed Classes
Missed classes for any reason (including illness, travel, or schedule conflicts) are not eligible for refunds or credits
Make-up tokens are the only accommodation for missed classes

Withdrawal & Cancellation Policy
A 30-day written notice via email is required to cancel enrollment
Verbal notice, text messages, or missed attendance will not be accepted as cancellation
Tuition will continue to be billed during the 30-day notice period
If proper notice is not given, I understand that I am responsible for any outstanding tuition

Behavior & Participation Expectations
My child is expected to follow all class rules and safety instructions
Disruptive, unsafe, or disrespectful behavior may result in removal from class

BIO Gymnastics reserves the right to discontinue participation for any athlete whose behavior impacts safety or the learning environment

In consideration for being allowed to participate in activities at BIO Gymnastics, I hereby release, waive, discharge, and covenant not to sue BIO Gymnastics, its owners, directors, officers, employees, coaches, contractors, volunteers, and affiliated parties from any and all liability, claims, demands, or causes of action arising out of or related to any loss, damage, or injury that may be sustained.
This includes, but is not limited to, injuries resulting from negligence to the fullest extent permitted by law.
