By registering, I acknowledge that I have read, understand, and agree to follow the Leap of Joié Dance Studio Studio Policies available on the studio website.

I, the undersigned parent or legal guardian, give permission for my child to participate in classes, rehearsals, performances, camps, workshops, and other activities offered by Leap of Joié Dance Studio.

I understand that dance is a physical activity involving movement, stretching, jumping, turns, and other exercises that carry inherent risks of injury, including but not limited to sprains, strains, bruises, fractures, and other injuries. I voluntarily assume these risks on behalf of my child.

I certify that my child is physically able to participate in dance activities and agree to inform Leap of Joié Dance Studio of any medical conditions, allergies, injuries, or physical limitations that may affect my child's participation.

In the event of an emergency, I authorize Leap of Joié Dance Studio and its staff to obtain reasonable emergency medical care for my child if I cannot be reached. I understand that I am responsible for any medical expenses incurred.

I release and hold harmless Leap of Joié Dance Studio, its owner, instructors, assistants, employees, volunteers, and the facility where classes are held from liability for injuries, accidents, illnesses, loss, theft, or damage to personal property that may occur while my child is participating in studio activities, except where prohibited by applicable law.

I understand that participation may be suspended if my child has a contagious illness or medical condition that could place others at risk.

By accepting this waiver, I acknowledge that I have read, understood, and voluntarily agree to the terms of this Liability Waiver and Assumption of Risk or behalf of myself and my child.

I certify that I have provided accurate medical and emergency contact information for my child.

I understand that it is my responsibility to notify Leap of Joié Dance Studio of any changes to my child's medical conditional, allergies, medications, emergency contacts, or health information.

In the event of an illness or injury during studio activities, I authorize Leap of Joié Dance Studio staff to provide reasonable basic first aid within the scope of their training.

If my child requires emergency medical treatment and I cannot be reached immediately, I authorize Leap of Joié Dance Studio to contact emergency medical services (911) and obtain necessary emergency medical care for my child.

I understand that every reasonable effort will be made to contact me or my designated emergency contact as quickly as possible.

I acknowledge that I am financially responsible for any medical treatment, transportation, or related expenses incurred on behalf of my child.

By accepting this agreement, I authorize the emergency medical treatment described above and acknowledge that I have read and understand this Medical Authorization and Emergency Treatment Consent.

I acknowledge that I have read, understand, and agree to follow the Leap of Joié Dance Studio Parent Handbook and Studio Policies.

I understand that the Parent Handbook includes, but is not limited to, information regarding:
Tuition and payment policies
Registration fees
Attendance and make-up classes
Dress code
Drop-off and pick-up procedures
Student and parent expectations
Health and safety policies
Studio communication
Recitals, performances, and special events

I understand that the Parent Handbook is available electronically on the Leap of Joié Dance Studio website and may be updated from time to time. I agree to review any updates and abide by all current studio policies.

By accepting this agreement, I acknowledge that I have read and understand the Parent Handbook and agree to comply with all studio policies and procedures.

I grant permission for Leap of Joié Dance Studio to photograph and/or record my child during classes, rehearsals, performances, studio events, and other studio activities.

I understand that these photographs and videos may be used for educational, promotional, and marketing purposes, including but not limited to:

The Leap of Joié Dance Studio website
Social media platforms
Printed promotional materials
Advertisements
Newsletters
Studio displays

I understand that no compensation will be provided for the use of these images or recordings.

If I do not wish for my child's image or likeness to be used, I understand that I must notify Leap of Joié Dance Studio in writing before my child's participation begins. The studio will make reasonable efforts to honor this request.

By accepting this agreement, I acknowledge that I have read and understand this Photography and Video Release.
