By creating an account, I confirm that I am over the age of 18, and I am the parent/legal guardian of the student(s) being registered. I agree to follow studio policies regarding tuition, attendance, communication, and participation.
I/We understand that there is a certain amount of inherent risk involved in the activity of dancing. In consideration of the benefits derived from the Academy of Performing Arts on Broadway, I/we hereby agree to hold harmless the Academy of Performing Arts on Broadway, its agents or employees from any and all claims for personal injury to myself, my/our child while participating in said dance or any other Academy of Performing Arts activities, or while being transported to and from said activities related to dance or any other Academy of Performing Arts activities. I/we understand and give consent for the use of my child's picture for promotional use on the studio website/social media, and/or posting in the studio. I understand no personal information will be used. I give consent to receive email notifications from the Academy. I understand these are important notifications and need to be read. The Academy will observe all public and school holidays. All fees are non-refundable. Any student whose conduct conflicts with the Academy's standards will be dismissed from their class. I have read and agree to this waiver.
