PARTICIPATION RELEASE:
I hereby represent that I am the parent or legal guardian of the “athlete”. I hereby acknowledge that I am familiar with the physical and otherwise athletic nature of cheerleading and all activities related thereto, including without limitations; jumping, tumbling, building of stunts and pyramids, dancing, and other activities related there to (collectively, “Cheerleading Activities”), and in connection with the athletes acceptance as a member of SOUTH FORCE CHEER, the Athlete will be participating in Cheerleading actives during classes, practices, performances, and competitions, in a variety of locations settings and venues.
In consideration of the acceptance of the Athlete as a member of SOUTH FORCE CHEER, in my capacity as the parent or legal guardian of the Athlete, I on behalf of myself and the Athlete, hereby consent to the Athlete’s participation in the Cheerleading Activities and hereby release and hold harmless SOUTH FORCE CHEER, its principles, partners, members, managers, employees, officers, contractors, consultants, advisors, volunteers, and agents from any and all actions, causes of actions, damages, liabilities and claims, relating to or in connection with the Athlete’s participation in Cheerleading Activities, including without limitations, actions, causes of action, damages, liabilities, and claims relating to bodily injury or illness, during my absence. I hereby authorize SOUTH FORCE CHEER Personnel to perform simple first aid on the Athlete if deemed necessary of advisable in the discretion of SOUTH FORCE CHEER Personnel, during my absence. I hereby represent that the Athlete is in good health and physically able to participate in Cheerleading Activities and that the athlete is and will continue to be covered by sufficient insurance to cover costs and expenses of injuries the occurrence of which are reasonably foreseeable from the Athlete’s participation in Cheerleading Actives. I authorize the necessary steps regarding medical attention (i.e. First aid, calling ambulance service, transportation to the hospital and to be admitted to the hospital) and will authorized hospital faculty and staff to treat my child for any illness or injury he or she has.
For any calendar month in which the athlete is enrolled as a member of SOUTH FORCE CHEER, I shall pay the tuition and fees required by SOUTH FORCE CHEER in connection the the Athlete’s enrollment as a member of SOUTH FORCE CHEER in accordance with the rules and regulations of SOUTH FORCE CHEER as established from time to time. Each installment of tuition, fees and dues shall be due and payable on the first day of each calendar month in which the Athlete is so enrolled (except in the case that the Athlete’s initial enrollment which occurs on a day other that the first day of the month, in which case, such initial installment of tuition and fees shall be due on the day of such enrollment).
A late fee in the amount of ten dollars shall be due and payable for each installment of tuition or fees not paid by the fifteenth day of that calendar month. I understand that if my account is not paid in full by the end of the final month of enrollment, it shall accrue interest at the rate of 1.5% per month (18% per year) on the unpaid balance. In the event that legal action should become necessary, I/we agree to pay all attorneys’ fees, court costs, filing fees, and collection costs.
PERMISSION FOR MEDICAL TREATMENT:
I authorize the necessary steps regarding medical attention (i.e. First aid, calling ambulance service, transportation to the hospital and to be admitted to the hospital) and will authorized hospital faculty and staff to treat my child for any illness or injury he or she has.
ALL STAR ATHLETES:
I have read the ALL STAR CHEER packet in its entirety.
I have read and agree with the Monthly Tuition Financial Obligation.
I have read and agree with the Competition Fee and Uniform Fee payments.
I have read and agree with the attendance expectations and policy.
I have read and will abide by the rules and regulations set forth by South Force Cheer.
I understand my commitment to competition dates and will attend all.
I understand if I terminate my membership after August 1st, 2026. I am required to pay a $200 termination fee.
I understand that all competition and uniform fees are non-refundable.
I understand I may communicate with coaches by email or by calling the gym.
Parents are to make sure their athlete is on time and attends all practices, events, and competitions.
Parents are to inform the head coach 24 hrs prior if the athlete will be late or absent from practice.
Parents are to fulfill all financial obligations on time; whoever signs the release form is financially responsible.
Parents are to encourage and support South Force to be the best they can be.
If parents cannot stay for the whole competition, other arrangements must be made.
Occasionally, additional practices and competitions may be added throughout the year as needed. These will be followed as per the attendance policy.
Parents need to maintain manners and civility at all functions. Unsportsmanlike behavior can result in a deduction on the team's scoresheet. Please show respect to competition officials and other fans. If any family member is seen speaking to Varsity official’s at competitions this will result in a fine or dismissal from the team.
Any negative behavior towards an SFC Team member or another gym (i.e., Social Media); will result in disciplinary action.
No inappropriate pictures or language on any account (i.e., Social Media).
Athletes and parents are never to approach an official or judge of a competition to voice a negative opinion.
Team members will set and maintain high examples of behavior and are constantly working to achieve personal and squad goals.
Team members must be aware that it is a privilege to be a part of South Force and be mindful of the responsibilities required and commitment to the team.
By signing, I agree to have read the entire packet in full and understand and will follow all standards, rules, and financial obligations set by SOUTH FORCE CHEER.
