Assumption of Risk and Release of Liability
Dance and physical movement activities involve inherent risks of injury. By enrolling in classes at Spark Dance Studio, the student (or parent/guardian on behalf of a minor student) acknowledges and agrees to all of the following:
1. I understand that participation in dance classes involves physical activity and carries an inherent risk of injury.
2. I voluntarily assume all risks associated with participation.
3. I release, waive, and discharge Spark Dance Studio, its owners, staff, and representatives from any and all claims, demands, or causes of action arising from injury, illness, loss, or damage sustained during participation, including travel to and from the studio.
4. I agree to inform Spark Dance Studio of any pre-existing medical conditions, physical limitations, or injuries that may affect the student's ability to participate safely.
Code of Conduct
Spark Dance Studio is committed to maintaining a safe, respectful, and inclusive environment for all students, families, and staff. All students and accompanying adults are expected to:
1. Treat instructors, staff, fellow students, and families with respect and courtesy at all times.
2. Arrive on time and prepared for class in appropriate dance attire and footwear.
3. Refrain from disruptive, aggressive, or unsafe behavior in the studio or on studio premises.
4. Respect studio equipment, property, and the personal belongings of others.
5. Follow all instructor directions promptly and cooperatively.
6. Refrain from the use of cell phones or other devices during class.
Parents and guardians are expected to model respectful behavior and are not permitted to interrupt or enter the classroom during instruction unless invited by the instructor or in the case of an emergency.
Spark Dance Studio reserves the right to dismiss any student or family member whose conduct is deemed disruptive or unsafe, without refund of tuition.
Medical Emergency Authorization
In the event of a medical emergency, and if the student's parent or emergency contact cannot be reached in a timely manner, I hereby authorize Spark Dance Studio staff to:
Contact emergency medical services (911) on behalf of the student.
Consent to emergency medical treatment as deemed necessary by medical professionals.
Spark Dance Studio staff are not medical professionals and are not responsible for medical decisions made by emergency responders.
Payment and Refund Policy
TUITION & FEES
Tuition is an annual fee split into 10 equal monthly installments from August through May of each season. Payments are the same each month regardless of holidays, absences, weather cancellations, or any other alterations or adjustments to the schedule. Tuition is due on the 1st of each month. Spark Dance Studio reserves the right to assess a late fee for payments received after the 10th of the month. Payment can be made online or in person with check or cash.
REFUNDS
Tuition is non-refundable. Tuition will continue to be billed on schedule unless we are notified directly that a student has dropped class(es). If we are notified that your dancer is dropping class before October 15, you are eligible for a refund of the Costume Fee ONLY if your balance is paid in full at the time of dropping.
Photo and Video Release
From time to time, Spark Dance Studio may photograph or record classes, rehearsals, performances, or studio events for use in promotional materials, social media, the studio website, or other marketing purposes. By agreeing to this waiver, you grant Spark Dance Studio permission to use the student's likeness, image, and/or voice in photographs and video recordings for promotional and educational purposes, without compensation. This waiver is NOT required and may be revoked at any time by logging into the studio portal, clicking the "Waivers" button and completing the Photo Opt-Out.
