Little Legends Dance Studio, a program of This Mom-ish LLC: Participation Waiver and Release I am the parent or legal guardian of the child registered. I understand that dance and movement activities involve physical activity and carry a risk of injury. I confirm my child is in good health and able to take part, and I will tell the instructor about any medical conditions, allergies or special needs. I voluntarily allow my child to participate and, to the fullest extent allowed by law, I release This Mom-ish LLC, Little Legends Dance Studio, its instructors, staff and the host studio from claims for injury, illness or loss arising from my child's participation, except in cases of gross negligence or willful misconduct. In an emergency, I authorize staff to seek medical care for my child if I cannot be reached, and I accept responsibility for any related costs. A parent or guardian (or an adult I designate) will stay on site during class and will pick up my child promptly. Photos and videos: I allow This Mom-ish to use photos or videos of my child taken during class for program and social media use. I may opt out at any time by emailing info@thismomish.com. Payments: tuition and fees are due as scheduled. Card processing fees are added to online payments. Refunds and credits follow the studio policy shared at registration.
