SIERRA ACADEMY OF DANCE Student Agreement
& Liability Waiver

I, the undersigned (parent/legal guardian if student is a minor), agree to the following:

1. Payment & Fees
I agree to pay all tuition, fees, deposits, and costume costs according to Sierra Academy of Dance policies. Tuition is due in advance and is non-refundable. No refunds or credits will be given for missed classes or withdrawal.

2. Photo & Media Release
I grant permission for Sierra Academy of Dance to photograph, record, and use images, video, or audio of the student for promotional or educational purposes without compensation

3. Assumption of Risk
I understand that dance and related activities involve physical activity and risk of injury. I voluntarily assume all risks associated with participation.

4. Health & Participation
I confirm that the student is physically able to participate and has no known condition that would prevent safe involvement, or I have disclosed such conditions.

5. Release of Liability
To the fullest extent permitted by California law, I release and hold harmless Sierra Academy of Dance, its owners, employees, and agents from any claims, injuries, damages, or liabilities arising from participation in studio activities, except in cases of gross negligence or willful misconduct.

6. Agreement
I have read and understand this agreement and agree to all terms.
