WAIVER AND RELEASE OF LIABILITY
South Georgia Ballet
This Waiver and Release of Liability ("Agreement") is entered into by and between the undersigned participant ("Participant") and South Georgia Ballet ("SGB").
1. Acknowledgment of Risk
Participant acknowledges that participation in dance classes, rehearsals, performances, and related activities involves inherent risks, including but not limited to physical injury, falls, strains, sprains, or other bodily harm. Participant voluntarily assumes all such risks associated with participation.
2. Medical Fitness
Participant affirms that they are physically fit and have no medical condition that would prevent safe participation. Participant agrees to inform SGB of any relevant medical conditions prior to participation.
3. Release and Waiver
In consideration for being allowed to participate in SGB activities, Participant hereby releases, waives, and discharges South Georgia Ballet, its directors, officers, employees, instructors, volunteers, and affiliates from any and all liability for injury, loss, or damage arising out of or related to participation, whether caused by negligence or otherwise, to the fullest extent permitted by law.
4. Indemnification
Participant agrees to indemnify and hold harmless South Georgia Ballet from any claims, demands, or causes of action brought by or on behalf of Participant, including reasonable attorneys’ fees.
5. Emergency Medical Treatment
Participant authorizes South Georgia Ballet to obtain emergency medical treatment on their behalf if necessary and agrees to be responsible for any associated costs.
6. Photo and Media Release
Participant grants permission to South Georgia Ballet to use photographs, video recordings, or other media taken during participation for promotional, educational, or marketing purposes without compensation.
7. Code of Conduct
Participant agrees to follow all rules, policies, and instructions provided by South Georgia Ballet staff and acknowledges that failure to do so may result in removal from activities.
8. Governing Law
This Agreement shall be governed by and construed in accordance with the laws of the State of Georgia.
9. Severability
If any portion of this Agreement is found to be invalid or unenforceable, the remaining provisions shall continue in full force and effect.
10. Acknowledgment and Signature
Participant acknowledges that they have read this Agreement, fully understand its terms, and sign it freely and voluntarily.
