# Dance Studio Participation Waiver and Release of Liability

## Assumption of Risk

I understand that participation in dance classes, rehearsals, performances, workshops, fitness activities, competitions, and related events involves inherent risks, including but not limited to slips, falls, collisions, muscle strains, sprains, fractures, and other injuries. I voluntarily choose to participate and assume all risks associated with these activities.

## Medical Fitness

I confirm that I am physically able to participate in dance activities. I understand that it is my responsibility to consult a healthcare provider if I have any concerns regarding my health or ability to participate.

## Release of Liability

To the fullest extent permitted by applicable law, I release and hold harmless **[Dance Studio Name]**, its owners, directors, instructors, employees, contractors, volunteers, and agents from claims arising out of ordinary negligence related to my participation in studio activities. This release does not apply to liability that cannot legally be waived under applicable law, including gross negligence or intentional misconduct where prohibited by law.

## Medical Treatment Authorization

In the event of an emergency, I authorize the studio to obtain appropriate medical treatment for me (or my child) if I or my emergency contact cannot be reached. I understand that I am responsible for all medical expenses incurred.

## Personal Property

I understand that the studio is not responsible for the loss, theft, or damage of personal belongings brought onto the premises.

## Cancellation and Refund Policy Acknowledgment

I acknowledge that I have read and understand the studio's cancellation and refund policies.

* Registration fees and membership fees are non-refundable unless otherwise stated in writing.
* Tuition reserves a participant's place in class and is not refunded for missed classes, withdrawal, dismissal, or unused lessons except as outlined in the studio's published policies.
* Requests to withdraw from classes must be submitted in writing. Any refunds or account credits will be issued only in accordance with the studio's current withdrawal policy.
* If the studio cancels a class due to circumstances within its control, a make-up class, studio credit, or refund may be offered at the studio's discretion.
* In the event of circumstances beyond the studio's reasonable control (including severe weather, power outages, public health directives, or other emergencies), classes may be rescheduled, moved online, or otherwise adjusted. Refunds may not be provided in these circumstances.

## Missed Class Policy

I understand and agree to the following:

* Tuition is based on enrollment, not attendance.
* Missed classes do not automatically qualify for refunds or credits.
* Make-up classes, if offered, must be scheduled according to studio availability and policies.
* It is the responsibility of the participant or parent/guardian to notify the studio if a class will be missed.
* The studio is not responsible for providing make-up classes for absences due to vacations, personal scheduling conflicts, illness, or other reasons unless otherwise stated in the studio's policies.

## Photography and Video Consent (Optional)

I grant permission for photographs and videos taken during classes, rehearsals, performances, competitions, or studio events to be used by the studio for promotional, educational, website, print, and social media purposes.

I certify that I have carefully read this Dance Studio Participation Waiver and Release of Liability, understand its contents, and sign it voluntarily. I acknowledge that I have had the opportunity to ask questions before signing and agree to be bound by its terms.
