1. Acknowledgment of Risk

I understand that participation in cheerleading, dance, tumbling, and related athletic activities involves inherent risks, including but not limited to:

Falls, collisions, and physical contact
Sprains, strains, fractures, or more serious injuries
Equipment-related injuries
Injury resulting from the actions of other participants

I acknowledge that these risks may result in serious injury, permanent disability, or death, and I voluntarily accept full responsibility for these risks.

2. Assumption of Risk

I knowingly and willingly assume all risks associated with participation in any programs, classes, rehearsals, events, or activities at Glo Xtreme Cheer & Dance (“Glo Xtreme”), whether such risks are known or unknown.

3. Release & Waiver of Liability

In consideration of being allowed to participate, I hereby release, waive, and discharge:

Glo Xtreme Cheer & Dance, its owners, directors, coaches, staff, contractors, volunteers, and facility providers

from any and all liability, claims, demands, or causes of action arising out of or related to:

Injury, illness, or damage sustained during participation
Negligence (to the fullest extent permitted by Texas law)

4. Medical Authorization

I certify that the participant is physically fit to participate. In the event of an emergency, I authorize Glo Xtreme staff to:

Obtain emergency medical treatment
Contact emergency services (911)
Secure transportation to a medical facility

I understand that I am financially responsible for any medical care provided.

5. Illness & Communicable Diseases

I understand that participation may expose the athlete to illnesses, including but not limited to communicable diseases. I voluntarily assume this risk.

6. Safety Rules & Conduct

I agree that the participant will:

Follow all gym rules, safety procedures, and coach instructions
Wear proper attire and footwear
Refrain from unsafe behavior

Failure to follow rules may result in removal from activities without refund.

7. Indemnification

I agree to indemnify and hold harmless Glo Xtreme from any claims brought by or on behalf of the participant, including legal fees and costs.

8. Governing Law

This agreement shall be governed by the laws of the State of Texas.

9. Acknowledgment & Agreement

By registering for or participating in any activities with Glo Xtreme Cheer & Dance, and by selecting the acknowledgment checkbox during registration, I certify that:

I have read and fully understand this Liability Waiver & Release of Claims
I understand that I am waiving certain legal rights, including the right to sue
I agree to all terms voluntarily and without coercion
I am the parent/legal guardian of the participant, or I am the participant and at least 18 years of age

I understand that this agreement is legally binding and applies to all current and future participation unless revoked in writing.

I grant permission for Glo Xtreme Cheer & Dance (“Glo Xtreme”) to photograph and record video of the participant listed on this account.

I understand and agree that these images and recordings may be used for:

Social media
Marketing and advertising
Website and promotional materials
Print and digital publications

I acknowledge that:

No compensation will be provided for use of these images or videos
Glo Xtreme retains all rights to the content
Images and videos may be used in future promotional materials without additional notice

I release and hold harmless Glo Xtreme Cheer & Dance, its owners, staff, and affiliates from any claims related to the use of these materials.
