Peak Music and Dance Enrollment Agreement

Enrollment at Peak Music and Dance is a commitment to a structured training program designed
for real progress.

By enrolling in classes at Peak Music and Dance, you acknowledge and agree to the following:

Tuition & Payment
All accounts are enrolled in automatic billing. Tuition is charged on a recurring basis based on
your enrollment. If a payment fails, it will be retried. A 7-day grace period will be provided to
resolve the issue. If payment is not completed within that time, a $15 late fee may be applied.
Accounts must remain in good standing for continued participation.

Attendance & Make-Ups
Regular attendance is essential for progress. Missed classes are not refunded. Make-up classes
may be offered when a comparable class is available, but are not guaranteed.

Withdrawal & Cancellation
A minimum of 14 days written notice via email is required to cancel enrollment. Tuition will
continue during the notice period.

Refund Policy
Tuition is non-refundable. In rare circumstances, a studio credit may be issued at the sole
discretion of Peak Music and Dance.

Liability Waiver
Participation in music and dance training involves physical activity. By enrolling, you
acknowledge and accept all associated risks and agree that Peak Music and Dance is not liable
for injuries that may occur during participation.

Media Release
Unless otherwise requested in writing, you grant permission for Peak Music and Dance to
photograph or record students and to use these materials for promotional purposes, including
social media, website, and marketing.

Studio Conduct & Community Standards
Peak Music and Dance is a professional, inclusive environment grounded in respect, discipline,
and support. We do not tolerate disrespect, disruptive behavior, or intolerance. Participation
requires adherence to these standards by students and family members.

Policy Agreement
Enrollment constitutes agreement to all studio policies. Policies may be updated as needed to
support operations, safety, and the student experience.
