FULL OUT TC
RELEASE OF LIABILITY & WAIVER AGREEMENT

By registering through GoStudioPro for participation in programs, classes, camps, clinics, open gyms, events, or private lessons at Full Out Training Center (“Full Out TC”), I acknowledge and agree to the following:

1. Assumption of Risk

I understand that participation in cheerleading, tumbling, gymnastics, strength training, conditioning, open gym activities, and related athletic activities involves inherent risks. These risks may include, but are not limited to:

Falls from height

Collisions with equipment or other participants

Sprains, strains, fractures, or other serious injuries

Permanent disability or death

I voluntarily assume all risks, both known and unknown, associated with participation in activities at Full Out TC.

2. Release and Waiver of Liability

In consideration of being permitted to participate, I hereby release, waive, discharge, and hold harmless Full Out Training Center, its owners, directors, coaches, employees, contractors, volunteers, and affiliates from any and all claims, demands, causes of action, damages, or liability arising out of or related to participation in any Full Out TC activity, whether caused by negligence or otherwise, to the fullest extent permitted by law.

3. Medical Authorization

I certify that the participant is physically able to participate in activities at Full Out TC.

In the event of injury or medical emergency, I authorize Full Out TC staff to seek emergency medical treatment on behalf of the participant. I understand that I am responsible for any medical expenses incurred.

4. Photo & Media Release

I grant Full Out Training Center permission to use photographs and/or video recordings of the participant for promotional, marketing, website, and social media purposes without compensation.

If you do NOT consent to photo use, you must notify Full Out TC in writing.

5. Payment & Billing Acknowledgment

I understand that registration and billing are processed through GoStudioPro. By enrolling:

I agree to the tuition rates and fees listed.

I authorize recurring charges if enrolled in ongoing programs.

I understand that fees are non-refundable unless otherwise stated in writing.

I agree to provide updated payment information as needed.

6. Code of Conduct

Participants and parents agree to follow all gym rules, safety policies, and staff instructions. Full Out TC reserves the right to suspend or terminate participation for unsafe behavior, misconduct, or failure to follow policies without refund.

7. Acknowledgment of Agreement

By completing registration in GoStudioPro, I acknowledge that:

I have read this Release of Liability & Waiver Agreement in full.

I understand its terms.

I voluntarily agree to its conditions.

I am legally authorized to sign on behalf of the participant if under 18 years of age.
