By signing below (including electronic signature), I acknowledge that I have read, understood, and voluntarily agree to all terms and conditions of this Waiver, Release, and Studio Policies Agreement.

I understand that participation in dance classes, rehearsals, workshops, intensives, performances, competitions, and related activities at J&M Youth Ballet Academy involves inherent risks, including but not limited to muscle strains, sprains, falls, collisions, physical injury, or other unforeseen injuries.



I voluntarily assume all risks associated with my child’s participation in activities at J&M Youth Ballet Academy, whether caused by my child, other participants, instructors, staff, or the condition of the premises.

I understand that:

refunds will not be issued for special events, workshops,intensives, or camps unless canceled by the studio.
J&M Youth Ballet Academy reserves the right to cancel or reschedule classes if necessary.

I understand that students are expected to behave respectfully toward instructors, staff, and fellow students. Disruptive, unsafe, or inappropriate behavior may result in removal from class or dismissal from J&M Youth Ballet Academy without refund.



Parents and guardians are expected to communicate respectfully with studio staff at all times.

I confirm that my child is physically capable of participating in dance activities and does not have any medical condition that would prevent safe participation, except as disclosed in writing to J&M Youth Ballet Academy.



I agree to inform the studio of any changes in my child’s medical condition.

I hereby release, waive, and discharge J&M Youth Ballet Academy, its owners, directors, instructors, employees, volunteers, contractors, landlords, and agents from any and all claims, demands, actions, or causes of action arising out of or related to any injury, illness, damage, or loss sustained by my child while participating in studio activities, including those occurring on the premises or during studio-sponsored events.



This release applies to claims based on negligence to the fullest extent permitted by law.

I understand that parents/guardians are responsible for timely drop-off and pickup of their child. J&M Youth Ballet Academy is not responsible for students left unattended outside of scheduled class times.

In the event of an accident or medical emergency, I authorize J&M Youth Ballet Academy and its representatives to obtain emergency medical treatment for my child if I cannot be reached.



I understand that I am financially responsible for any medical care or treatment provided.

I understand that placement in classes, performances, competitions, or intensives is at the discretion of J&M Youth Ballet Academy and its instructors, based on skill level, attendance, behavior, and readiness.

I grant permission to J&M Youth Ballet Academy to photograph and/or record my child during classes, rehearsals, performances, and events, and to use such photos or videos for promotional purposes, including but not limited to the studio website, social media, marketing materials, and advertisements.



I understand that no compensation will be provided for the use of such media.
