DTA Medical Release and Liability Release Form I understand that by taking part in this activity there is a possibility of injury or exposure to sickness/viruses to my daughter/son; therefore, with this knowledge I give permission for my daughter/son to participate in this activity, and do hereby grant permission to hospital staff members to administer immediate treatment to my child, should she/he become injured or ill. I also hold harmless Dancer’s Turnout Academy and their directors, instructors, employees, or affiliates, for any injury or sickness incurred as a result of my daughter’s/son’s participation in dance activities even if it is shown that they are negligent. Covid-19 exposure: By signing this document you understand while DTA has followed all necessary precautions, possible exposure can occur anywhere and you will not hold DTA responsible for any possible exposures to Covid-19. Tuition & Billing Policy – 2025–2026 Season All tuition is billed monthly through our secure billing system. Monthly Tuition Tuition is based on classes per week and is billed on the 1st of each month from September through May. Tuition remains the same regardless of the number of weeks in a month. Autopay Requirement All families are required to keep a valid card or ACH account on file. Tuition will automatically process on the 1st of each month. Failed Payments If a payment declines: • The system will automatically retry • A late fee may be applied after 5 days • Accounts 10+ days past due may result in removal from classes We understand things happen — communication is key. Unlimited Plans Unlimited memberships include up to 10 classes per week. Additional classes beyond 10 are $10 per class. Competition Packages Competition pricing is available only to approved competition team members and is not available for public enrollment. Drop-In Classes Single drop-in classes are $25 and must be paid prior to attending class. Private Lessons Private lessons are scheduled directly with the studio and must be prepaid.
