AiM does not carry medical insurance for its students. Although all activities are performed with the students' safety as the utmost concern, all students are required to be covered under their own family's insurance policies. If an injury should occur, you will only be compensated through your own family insurance policy. I understand that there is a risk of injury to my child with the participation in any dance class or rehearsal. I certify my child is in good health and able to participate fully in physical activity such as dance. I understand that I am waiving my right to bring legal action and to assert any claims against Arts in Motion, LLC, its respective agents or employees, for any and all negligence, loss, liability, damage, or personal injury, including death, that may be sustained by the participant, while in or upon the premises or any premises under the control of Arts in Motion, LLC, its owners or agents, or en route to or from any said premises.

I do hereby grant permission to Arts in Motion, LLC, and their respective agents and employees to seek medical aid and hospital services as they deem necessary for the child noted on this form in the event he/she should sustain injury or illness while attending Arts in Motion classes and rehearsals. I realize that every effort will be made to contact me first in the event of a medical emergency involving my child, and agree to indemnify and hold harmless AiM's agents in seeking medical care for my child. I have also indicated on this form my child's doctor and his/her contact information, a non-parent emergency contact, and any medical information of which Arts in Motion should be aware in consideration of the child's physical and mental well being.

I do hereby grant permission to Arts in Motion (AiM) to photograph my child or children. My child will not be identified if his or her image is used for promotional purposes. If photographed or filmed, my child's image may possibly be reproduced for AiM promotional media at any time in any form, including but not limited to print, digital, and electronic publishing via the Internet, without compensation.

Withdrawals: For school year classes only, we allow class withdrawals anytime before March 1st using our Withdrawal Form, which you can request by contacting our office. To avoid being charged on the 15th for the next tuition installment, the form must be submitted no later than the 1st of the month.

Please note: email, phone, or verbal withdrawal requests cannot be accepted — we must receive the official form. Accounts must be in good standing to process a withdrawal.

Because our school-year session runs through June, any withdrawal before the end of the season will result in the forfeiture of your pre-paid half-month June deposit. We are unable to offer pro-rated refunds for missed classes or unused tuition, except in the rare case of a class cancellation by Arts in Motion.

Transfers: We know schedules can change and dancers may need to change classes from time to time! Families are welcome to transfer to a different class through March 1st, depending on space and availability. Please stop by the front desk for help with a transfer.

After March 1st, transfer requests are handled on a case-by-case basis, depending on class space, recital preparation, and other program factors. We’ll always do our best to accommodate you whenever possible.

Arts in Motion makes every effort to hold each class offered. AiM reserves the right to reschedule, modify, or cancel dance classes and/or replace the instructor as the studio may deem necessary. AiM also reserves the right to deliver dance classes through an online platform in the event they cannot be conducted in-person for any reason, but not limited to: inclement weather, instructor absence, or loss of power, water or ventilation. In the event that a class does not reach the minimum number of students or the number falls below that minimum, AiM reserves the right to cancel or combine any class at any time. If such an instance should occur, you will be notified and we will discuss suitable alternatives. If none can be agreed upon, AiM will refund tuition payments in full. If the class has already begun, AiM will refund tuition for unused classes.

Studio Conduct Policy
Arts in Motion is committed to providing a welcoming, safe, and respectful environment for all students and families. Please help us maintain a positive space by following these guidelines:

Supervision
For everyone’s safety, children must remain with their parent or guardian when not in class. Please ensure your child stays in designated areas and does not bang on glass windows or mirrors, climb furniture, run, or play unsupervised.

Studio Spaces
Street shoes are not permitted in any of the dance studios. Water is the only beverage allowed in the studios. Food and other drinks are permitted only in common areas such as the lobby and student lounge.

Cleanliness
Please clean up after yourself and your child in all common areas, including the lobby, restrooms, and student lounge. Let’s work together to keep our shared spaces clean and welcoming.

Safety and Comfort
To ensure a child-friendly and comfortable environment for all families:

• Pets (except service animals), weapons of any kind, and prohibited substances are not permitted on studio premises.
• Individuals under the influence of alcohol or drugs may not remain on the premises.
• Out of respect for our shared indoor environment, we ask that guests refrain from entering the studio with strong or disruptive odors, including smoke or vapor from any source.

Arts in Motion reserves the right to ask any individual to leave the premises if their behavior, condition, or presence compromises the safety, comfort, or well-being of others.

Guests & Studio Use
We love that families and friends want to support our dancers! To ensure a comfortable and safe environment for all students, studio space is reserved for currently enrolled families during scheduled class times. Due to limited seating and capacity, we ask that families limit accompanying guests and avoid bringing extended groups to regular weekly classes. The lobby and common areas are intended for brief waiting periods only and are not designed for extended gatherings. Arts in Motion reserves the right to limit guest attendance or ask non-enrolled visitors to step outside if space becomes crowded or disruptive.
