My child has permission to participate in Studio 4 the Arts, LLC programs. I waive the right to claim against Studio 4 the Arts, LLC staff and teachers in the event of accident, injury, or loss of personal items. I understand that attending in person classes may pose a heightened risk of exposure to COVID-19 and I waive the right to make any claims agains Studio 4 the Arts due to exposure. I understand that Studio 4 retains the right to remove any student or parent from their program due to behavior issues or excessive unpaid fees. By signing this waiver, I agree to these terms and conditions set forth by Studio 4 the Arts, LLC.
