I/We realize that participation in dance classes and activities could result in some possible personal injury. Despite precautions being taken by the studio, accidents and injuries may occur. By agreeing to this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Step It Up Dance Cooperative.

I/We agree to report my child(ren)’s absences to the instructor(s) or Step It Up Dance Co board using BAND or Studio Pro prior to class start time.

Competitive dancers:
Excessive absences throughout the season or 3 absences after Christmas break of the same genre of dance may be subjected to a review of the dancer’s eligibility to compete. The Board Executive, in consultation with the instructor(s), may remove that dancer from a dance if it is felt that the commitment is not met.
Similarly, if a dancer does not attend any of the mandatory competitions, the Board Executive, in consultation with the instructor(s), may remove that dancer from any further classes, competitions and recital for the season.
If a dancer is removed, the family is responsible for all associated costs including but not limited to tuition, costumes and lost competition fees. The dancer will remain under review and may be asked not to return to Step It Up Dance Co.

Step It Up Dance Co accepts payments via e-transfers to stepitupdanceco@gmail.com, cheques, or cash.

Tuition is due when classes begin but we do understand this may not be possible for all families. Tuition can be divided into 2 payments, 1 in September and the other in January. Any other arrangements must be made with the Step It Up Dance Treasurer and a payment agreement must be signed. You will find this form on our website.

All accounts must be up to date by April 1st.

If Step It Up Dance Co incurs any NSF fees, this cost will be passed onto the family who issued the cheque.

I/We agree to release from responsibility the Step It Up Dance Cooperative of Clavet, Saskatchewan, including all teachers, dancers, volunteers, board members, and the facilities used by Step It Up Dance Cooperative from any cause of action claims or demands now and in the future. I/We will not hold Step It Up Dance Cooperative liable for any personal injury including, but not limited to scrapes, cuts, sprains, fractures, broken bones, concussions, death, or any personal property damage/loss, which may occur on the premises before, during, or after classes. Furthermore, I/we agree to obey the studio's code of conduct and facility rules and will take full responsibility for my/our behaviour in addition to any damage I/we may cause to the facilities utilized by Step It Up Dance Cooperative. I/we understand that Step It Up Dance Cooperative is a registered and insured non-profit organization. In the event that I/we should observe any unsafe conduct or conditions before, during, or after my/our classes, I/we agree to report the unsafe conduct or conditions to a teacher or board member as soon as possible.
