I understand that participation in dance classes, rehearsals, performances, workshops, events, and related activities with Pacific Legends involves physical activity and inherent risks, including but not limited to muscle strains, sprains, falls, collisions, or other injuries.

I voluntarily agree to allow myself and/or my child to participate in all activities offered by Pacific Legends and acknowledge that participation is at my own risk.
I hereby release, waive, and discharge Pacific Legends, its owners, directors, instructors, staff, volunteers, guest artists, and affiliated facilities from any and all claims, demands, causes of action, or liability arising out of or related to any injury, illness, loss, or damage that may occur during or as a result of participation in any Pacific Legends activities, whether occurring on or off studio premises.

I certify that I and/or my child are physically able to participate in dance activities and have no medical conditions that would limit safe participation, or that any such conditions have been disclosed to Pacific Legends.

I understand that Pacific Legends is not responsible for lost, stolen, or damaged personal property.
For participants under the age of 18, I affirm that I am the legal parent or guardian and that I consent to my child’s participation. I agree to assume full responsibility for any risk of injury or harm to my child associated with participation.

I acknowledge that I have read and fully understand this waiver, and I voluntarily agree to its terms.

I authorize Pacific Legends, its owners, directors, instructors, staff, and volunteers to obtain emergency medical treatment for myself and/or my child in the event of an illness or injury during participation in classes, rehearsals, performances, events, or related activities, when I cannot be reached.

I understand that reasonable efforts will be made to contact the parent/guardian or emergency contact listed before medical treatment is administered. However, I grant permission for emergency medical care to be provided as deemed necessary by qualified medical professionals.

I agree to assume full financial responsibility for any medical treatment, transportation, or services provided to myself and/or my child, including ambulance or hospital care.
I certify that I have disclosed all relevant medical conditions, allergies, medications, or physical limitations that may affect safe participation, or that no such conditions exist at this time.
I understand that Pacific Legends does not provide medical insurance coverage for participants.

For participants under the age of 18, I affirm that I am the legal parent or guardian and that I consent to this Medical Authorization & Emergency Consent on behalf of my child.
I acknowledge that I have read and fully understand this authorization and voluntarily agree to its terms.

I grant permission to Pacific Legends to photograph, video record, or otherwise capture images and recordings of myself and/or my child during classes, rehearsals, performances, events, and related activities.

I understand that these photos, videos, and recordings may be used for promotional, educational, and marketing purposes, including but not limited to social media, websites, printed materials, advertisements, and other media formats, without compensation.

I understand that Pacific Legends will use discretion and respect when selecting images and recordings and will not knowingly use content in a harmful or inappropriate manner.
I acknowledge that all images and recordings are the property of Pacific Legends and may be used indefinitely unless written notice of revocation is provided. I understand that revocation will apply only to future use and cannot retroactively remove previously published materials.

For participants under the age of 18, I affirm that I am the legal parent or guardian and that I consent to this release on behalf of my child.
I acknowledge that I have read and fully understand this Photo, Video & Media Release and voluntarily agree to its terms.

Pacific Legends is committed to protecting the privacy of our dancers and families. This policy explains how personal information is collected, used, and protected.

Information We Collect
-We may collect personal information including, but not limited to:
-Student and parent/guardian names
-Contact information (email, phone number, address)
-Emergency contact and medical information
-Photos and videos taken during classes, rehearsals, performances, or events

How Information Is Used
-Personal information is used solely for purposes related to:
-Class enrollment and scheduling
-Communication regarding classes, events, and studio updates
-Tuition billing and account management
-Emergency situations
-Studio marketing and promotional materials (as authorized through the Media Release)

Data Storage & Security
Pacific Legends uses DanceStudio-Pro to securely store and manage student and family information. Access to personal information is limited to authorized staff and instructors and is used only as necessary for studio operations.

Information Sharing
Pacific Legends does not sell, rent, or share personal information with third parties, except as required for:
-Payment processing
-Studio management software
-Emergency medical situations
-Legal or regulatory requirements

Communication
-By enrolling, families consent to receive studio-related communications via email or other electronic methods. Families may opt out of non-essential communications at any time.

Privacy of Minors
-We take special care to protect the privacy of minor participants. Personal information related to minors is used only for program operation and safety purposes.

Policy Updates
This privacy policy may be updated periodically. Continued participation in Pacific Legends constitutes acceptance of any updates.

I acknowledge that I have read, understand, and agree to the Privacy Policy & Data Use Acknowledgment outlined above.

I acknowledge and agree to follow the policies and expectations established by Pacific Legends to maintain a safe, respectful, and positive environment for all dancers, families, instructors, and staff.

Attendance & Participation
-Consistent attendance is expected for all classes, rehearsals, and performances.
-Absences should be communicated in advance whenever possible.
-Excessive absences may impact choreography, formations, or performance eligibility.

Behavior Expectations
-Dancers are expected to treat instructors, staff, fellow dancers, and volunteers with respect at all times.
-Disruptive, unsafe, or inappropriate behavior may result in disciplinary action, up to and including dismissal from class or the program.

Dress Code & Preparedness
-Dancers must arrive on time, dressed appropriately for class, with required attire, footwear, and hair secured.
-Proper attire is required for safety and effective participation.

Parent & Guardian Conduct
-Parents and guardians are expected to communicate respectfully with instructors and staff.
-Disruptive or inappropriate behavior by parents or guardians may result in removal from studio activities or events.

Studio Authority
-Pacific Legends reserves the right to make decisions regarding class placement, casting, choreography, scheduling, and performance participation in the best interest of the group.
-Failure to comply with studio policies may result in disciplinary action or dismissal without refund.

I understand and agree to the following tuition and payment policies established by Pacific Legends:

Tuition & Billing
-Tuition is billed on a monthly basis and is due according to the schedule established by Pacific Legends.
-Enrollment in classes constitutes a commitment to the full billing period.
-Tuition covers instruction, rehearsal time, and program participation and is not dependent on attendance.

Auto-Pay Authorization
-All families are required to maintain a valid payment method on file through the Pacific Legends parent portal.
-Tuition payments will be processed automatically on the designated draft date.
-Failed or declined payments may result in late fees and/or temporary suspension of participation until the account is brought current.

Late Payments & Fees
-Accounts not paid within 7 days of the due date will be considered late.
-A $15 late fee will be automatically applied to accounts past due after 7 days.
-Continued non-payment may result in suspension from classes or performances until the account is brought current.

No Refund Policy
-Tuition payments are non-refundable and non-transferable.
-No refunds or credits will be issued for missed classes, absences, or early withdrawal.

Returned Payments
-A fee may be applied for returned or failed payments.
-Families are responsible for any additional fees incurred due to declined transactions.

Program Changes
-Pacific Legends reserves the right to adjust class schedules, instructors, or programming as necessary.
-Tuition obligations remain in effect regardless of program changes.

I acknowledge that I have read, understand, and agree to the Tuition & Payment Policy outlined above.

I understand and agree to the following withdrawal and drop policies established by Pacific Legends:

-A 30-day written notice is required for any withdrawal from classes or programs.
-Notice of withdrawal must be submitted in writing via email or through the parent portal. Verbal notice will not be accepted.
-Tuition remains the responsibility of the family during the 30-day notice period, regardless of attendance.
-Failure to provide proper written notice will result in continued billing until notice is received and the notice period is fulfilled.
-Withdrawal from a class or program does not automatically terminate outstanding balances or financial obligations.
-No refunds or credits will be issued for withdrawals, missed classes, or early termination.

Pacific Legends reserves the right to adjust class placements, formations, or performance participation as a result of withdrawals.
