WAIVER AND RELEASE FROM LIABILITY FOR DANCE INSTRUCTION I have chosen to have my child/children, participate in dance instruction given by Move Makers Dance Studio. I acknowledge that I understand the nature of the activities my child will be participating in and that my child is in the proper physical condition and capable of participating in the related activities, understanding that the instructor is not in any way responsible for making such a determination. I acknowledge that I understand my child’s participation in any and/or all of the activities offered by Move Makers at this location may result in injury, loss or harm, even death. I waive any and all rights, claims and/or actions, in law or in equity, or personal injury and/or property damage, against Move Makers including its physical buildings and/or facilities, its/their owners, teachers, coaches, choreographers or trainers, or anyone else in any way connected therewith, in any way arising out of any physical presence on or in the premises and/or any studio activity whatsoever, whether on or away from either of the studio’s location, including but not limited to any or all classes, rehearsals, training sessions, competitions, performances, demonstrations, private lessons whatsoever. Move Makers requests that each student or participant consult with a physician before participating in any activity at or with Move Makers with respect to any past or present illness or injury that may affect his/her participation in, or ability to endure the exercise or dance activity, program, lesson or class.
