CODE OF CONDUCT
Students learn better in a secure, orderly, and non-disruptive environment. The goal of Mustard Seed Arts Academy is to help students, teachers, and volunteers create an environment that is conducive to teaching and learning.

Inappropriate or disruptive behavior by any student or accompanying parent, guardian, family member, or other caregiver will not be tolerated and may result in removal of the student from the class and/or termination from the institution.

A safe and positive learning environment for students and adults is essential to our success and the success of our students. We appreciate your commitment to these principles.

RULES
1. Listen politely and follow directions.
2. Keep hands, feet, and objects to yourself.
3. Be courteous and polite. Respect the rights of others and respect the property of Mustard Seed Arts Academy.
4. Use of appropriate language is expected at all times.

EXPECTATIONS FOR STUDENT BEHAVIOR
1. Students will arrive to class on time, will be dressed in appropriate attire for the class they are attending, and have the required supplies.
2. Students will listen carefully and respectfully to their instructors, volunteers, and administrators.
3. Students under the age of 18 will ask for assistance from the adults in charge when they have trouble with another student.

By signing below, the student and parent/guardian indicate their understanding and acknowledge agreement to abide by this Code of Conduct.

ILLNESS/INJURY POLICY

An ill student (fever, vomiting, etc.) should not attend class or rehearsals. This is for the safety of our students, instructors, and their families. Should a student come to class or rehearsal who appears to be ill, an instructor and/or administrator has the right to ask your child to leave class and send them home. A student is permitted return to class 24 after their fever has broken or symptoms have disappeared.

If a child/student has been injured outside of the classes or functions of Mustard Seed Arts Academy, requiring medical attention, documentation from a doctor on official letterhead must be presented to their instructor or an administrator to participate in Mustard Seed Arts Academy classes, rehearsals, and/or other functions. Documentation should clearly define any restrictions on their participation, if applicable. The instructor and/or director has the right to refuse instruction or ask students not to participate in classes, rehearsals, performances, and other functions if the instructor and/or administrator feels the student is still injured or the injury has not properly healed to participate in class.

If a student should be injured at Mustard Seed Arts Academy or one of its functions, an accident report will be completed and submitted to the Administration Office, and it will be at the discretion of the instructor/administration as to whether that student should continue with the class/lesson/rehearsal/performance, etc. or can be taught in the near future if a doctor’s excuse has not been submitted. If medical attention is required, the child/student will not be permitted to attend class again until a doctor’s slip is received by the instructor or administrator of Mustard Seed Arts Academy. Documentation should clearly define any restrictions on their participation, if applicable.

Please note, there are no refunds or make-ups for missed classes by the student regardless of the circumstances.

PHOTOGRAPHY/SOCIAL MEDIA/PUBLICITY MEDIA RELEASE

We love to promote our students and the activities they participate in at Mustard Seed Arts Academy. We use various media, including but not limited to written words, photographs, live and recorded videos, and other digital and still forms of communication.

We also understand the need for privacy in certain situations and wish to honor that.

I give my consent to Mustard Seed Arts Academy, to use my/my child’s name, likeness, image, voice and/or appearance to promote its activities in uses which may include, but are not limited to, press releases, illustrations, bulletins, exhibitions, videos, reprints, reproductions, publications, advertisements, and any promotional or educational materials in any medium now known or later developed. I agree that Mustard Seed Arts Academy has complete ownership of such pictures, video, etc., including the entire copyright, and may use them for any purpose consistent with Mustard Seed Arts Academy’s mission.
