1. Assumption of Risk
I acknowledge that dance and movement activities involve physical activity and inherent risks of injury. I understand that the student named above may experience injuries, including minor strains or more serious injury. I voluntarily assume all risks associated with participation in classes, rehearsals, performances, and other studio activities at El Dorado School of Performing Arts (“EDSPA”).
2. Release of Liability
I, on behalf of myself and the student named above, hereby release, discharge, and hold harmless EDSPA, its instructors, employees, and volunteers from any claims, demands, or causes of action arising out of participation in studio activities, including but not limited to personal injury, illness, or property damage, except as caused by gross negligence or intentional misconduct.
3. Medical Authorization
In case of injury or medical emergency, I authorize EDSPA staff to seek medical care for the student, including contacting emergency medical services. I understand I am responsible for any related medical costs.
4. Photo/Video Release
I grant permission for EDSPA to take photographs and videos of the student during classes, rehearsals, performances, and studio events. I agree that these images may be used for promotional purposes, including social media, website, print materials, or advertising. I understand that the student’s full name may or may not be used.
5. Health & COVID-19 Acknowledgment
I confirm that the student is in good health and able to participate in physical activity. I understand that EDSPA follows recommended health and safety guidelines but cannot guarantee prevention of illness, including COVID-19, and I voluntarily assume this risk.
