Health & Safety I understand that dancing is physical and sometimes injuries can happen (like falls, sprains, or sore muscles). I agree that my child (or I) is healthy enough to take part in dance classes I will let the studio know about any health issues, allergies, or injuries that could affect participation. I will keep my child home if they are sick, contagious, or showing symptoms such as fever or cough.Medical Care If there’s an emergency, I give permission for the staff to provide first aid and call emergency services if needed. I understand that I am responsible for any medical costs. Liability I understand that the instructors and staff will do their best to provide a safe environment, but accidents can still happen. I agree not to hold Marissa’s Dance Academy, its staff, or instructors responsible for any injuries, accidents, or illnesses that may occur while participating in dance classes or events.
At Marissa’s Dance Academy, we love celebrating our dancers and sharing their progress with our community. We regularly take photos and videos during classes, rehearsals, and performances. These may be used for: Studio newsletters, flyers, and printed materials Social media accounts and website Advertising or promotional purposes End-of-year slideshows or recital videos Privacy & Safety Children’s full names will not be used in any public post or promotional material. Studio staff and instructors will not post photos or videos of students on their personal social media pages without first getting parent/guardian permission. By signing below, I give permission for Marissa’s Dance Academy to take and use photos and/or videos of my child (or myself, if I am the student). I understand these images may be shared publicly by the studio and may be used without compensation. I release Marissa’s Dance Academy, its staff, and instructors from any claims or liability connected with the use of these images.
