I hereby grant the The Cave Athletics and its successor's permission to the unrestricted right to use the undersigned's name likeness, or appearence on any posters, calendars, photographs, try-out flyers, video material, film material, computer software, computer hardware, electonric on-line serices, or other similar promotional material in any form, content or medium to promote or market The Cave Athletics without payment in any form. The undersigned does hereby expressly release and wave any demand, action, claim, license, royalty, or other from of payment that undersigned, and his or her agents, representatives or assigns, may have based on claims of the undersigned as to rights of privacy, publicity, notoriety or any other rights arising out of or related to any use by The Cave Athletics LLC. of the undersigned's name, likeness or appearance.
I understand and agree that all photos will become the property of the The Cave Athletics and will not be returned.
I hereby irrevocably authorize the The Cave Athletics to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
I hereby hold harmless, release, and forever discharge the The Cave Athletics from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
