Embody Dance LLC – Release of Liability Waiver

1. Assumption of Risk
I understand that participation in dance, fitness, and performance activities involves physical movement and inherent risks of injury, including but not limited to muscle strains, sprains, falls, or other bodily harm. I voluntarily choose for myself or my child to participate in classes, rehearsals, workshops, and events offered by Embody Dance LLC and assume all risks associated with these activities.

2. Release and Waiver
I, on behalf of myself and/or my child, hereby release and hold harmless Embody Dance LLC, its owners, instructors, employees, volunteers, and affiliates from any and all claims, demands, damages, rights of action, or causes of action arising out of or connected with participation in any studio activity. This release includes, but is not limited to, claims of negligence, personal injury, property damage, or medical expenses.

3. Medical Authorization
In the event of injury or medical emergency, I authorize Embody Dance LLC staff to provide basic first aid and, if necessary, seek emergency medical care. I understand that every effort will be made to contact me prior to treatment and that I am responsible for any medical expenses incurred.

4. Photography and Media Consent
I understand that classes and events may be recorded or photographed for promotional purposes.

5. Acknowledgment of Understanding
I have carefully read and fully understand this Release of Liability Waiver and agree to its terms voluntarily. I understand that by signing this document, I am giving up certain legal rights, including the right to sue for injuries or damages that may result from participation.

Embody Dance LLC – Studio Agreement Form
Parent/Guardian Agreement & Liability Waiver

1. Studio Policies Acknowledgment
• Regular attendance is necessary for progress and participation in performances. Excessive absences may affect class placement or performance eligibility.
• Students arriving more than 10 minutes late may be asked to observe for safety reasons.
• All dancers must follow the studio’s dress code for their class level and style.
• Respectful behavior toward instructors, peers, and the studio space is expected at all times.
• Parents are responsible for checking studio emails and messages for updates, closures, and event notices.

2. Payment & Refund Policy
• Tuition is due on the 1st of each month and is considered late after the 5th, incurring a $15 late fee.
• No refunds for missed classes, but makeup classes may be offered when available.
• If a class is canceled due to weather or emergency, a rescheduled class or credit will be provided.
• Withdrawals require a 30-day written notice to avoid being charged for the next month.

3. Liability Waiver
I understand that participation in dance involves physical activity and potential risk of injury. I hereby release Embody Dance LLC, its instructors, staff, and affiliates from any and all liability for injuries, damages, or losses sustained while participating in any studio class, event, or performance. I certify that my child is physically fit to participate in dance classes.

4. Media Release
I grant permission for my child’s photograph or video to be used for studio promotional purposes, including social media, website, and marketing materials.

5. COVID-19/Health & Safety Acknowledgment
I agree to follow all health and safety guidelines implemented by Embody Dance LLC, including staying home if my child is ill.

6. Medical Release
In the event of a medical emergency, I authorize Embody Dance LLC staff to administer basic first aid and to contact emergency medical services if necessary. I understand that every effort will be made to contact me or the listed emergency contact before treatment. I agree to assume all financial responsibility for medical treatment incurred.
