RELEASE OF LIABILITY AND ASSUMPTION OF RISK
As a participant in dance classes with Susan Schwartz, I hereby state that I am in good physical condition and have no medical, physical, or psychiatric conditions that would jeopardize my participation. I acknowledge that dance training involves strenuous physical activity and I voluntarily assume all risks of personal injury, property damage, or disability that may arise from my participation, whether caused by the risks of the activity or by the ordinary negligence of the studio, its owner, its staff, or its representatives.
I hereby fully and forever release, acquit, and discharge Susan Schwartz, the Middle of Main Building, and their respective principals, officers, directors, employees, independent contractors, agents, insurers, representatives, successors, and assigns from any and all claims, actions, or causes of action of any nature whatsoever, whether known or unknown, arising out of my participation in classes or events at the studio.
I agree that I am solely responsible for any costs, medical expenses, or damages I may incur, and I covenant not to sue or assert any claim against the aforementioned parties. I have read, understand, and voluntarily accept these terms and conditions. By signing, I acknowledge that I am giving up substantial legal rights, including the right to sue.

In the event of a medical emergency during class time, I understand that Susan Schwartz or assigned studio personnel will make every reasonable effort to contact me immediately via the emergency contact information provided in my registration profile.
If I cannot be reached, I hereby authorize Susan Schwartz or a studio administrator to arrange for emergency medical treatment as deemed necessary by qualified medical professionals. I acknowledge that I am solely responsible for any and all costs, ambulance charges, and medical expenses incurred as a result of such treatment. I further authorize the disclosure of necessary medical information to medical personnel to facilitate immediate care.

For valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I irrevocably consent to and authorize Susan Schwartz (and those acting under her authority) to record, use, reproduce, and distribute any and all photographs, video images, or audio recordings of me or my child.
I understand and agree that these assets may be used for promotional, educational, or advertising purposes in any and all media now known or hereafter developed, including but not limited to, websites, social media platforms, print materials, and digital advertising, without further compensation to me.
All such images, recordings, and any derivatives thereof shall constitute the sole and exclusive property of Susan Schwartz. I waive any right to inspect or approve the finished product or the copy that may be used in connection therewith, and I waive any claims, including those based on defamation or invasion of privacy, arising out of or in any way connected to the use of such media.
