By submitting this form you are registering for classes with Hudson Conservatory of Ballet and agree to the terms below.

FINANCIAL AGREEMENT
I understand that registration fees, tuition, membership fees, performance fees, costume fees, and other studio charges are non-refundable unless otherwise stated in writing by Hudson Conservatory of Ballet (HCB).
Monthly tuition is due on the 1st of each month. Full Season and PAC tuition is billed September through May. PD tuition is billed August through May. Families are responsible for maintaining an active payment method and ensuring payments are made on time.
Missed classes due to vacations, family commitments, school activities, sports, illness, or other outside obligations are not eligible for refunds, credits, or prorated tuition. Students are encouraged to attend available make-up classes when appropriate.
Costume, performance, and production-related fees are non-refundable and serve as confirmation of a dancer's participation in the associated performance. Should a student withdraw after costume orders have been placed, all outstanding costume balances remain the responsibility of the family and must be paid in full prior to receiving any costumes purchased on the dancer's behalf.
Tuition adjustment requests due to extended illness, injury, or relocation may be considered on a case-by-case basis at the discretion of Hudson Conservatory of Ballet. Documentation may be required, and approved adjustments may be subject to applicable processing fees or non-recoverable transaction costs.
HCB reserves the right to adjust class schedules, faculty assignments, programming, and class offerings as needed. Classes must maintain minimum enrollment requirements to remain active.

I understand that participation in dance, movement, fitness activities, rehearsals, performances, camps, workshops, and related programs involves inherent physical risks, including but not limited to falls during turns, jumps, and lifts, partner-assisted movements, floor-impact and repetitive strain injuries, collisions, muscle strains, and other injuries.
On behalf of myself and my child(ren), I voluntarily assume all risks associated with participation in any Hudson Conservatory of Ballet activity.
To the fullest extent permitted by law, I release and hold harmless Hudson Conservatory of Ballet (HCB), its owners, directors, employees, contractors, volunteers, landlords, and representatives from any claims, liabilities, damages, costs, or expenses arising from participation in HCB programs, including claims based on ordinary negligence, except in cases of gross negligence or willful misconduct.
If any portion of this release purporting to bind a minor participant is found unenforceable under applicable law, the assumption of risk, release, and indemnification set forth above shall remain fully effective as to the claims of the undersigned parent or guardian.
HCB staff may administer basic first aid immediately and may seek emergency medical treatment without delay when reasonably necessary.
In the event of an emergency, I authorize HCB staff to obtain appropriate medical treatment for my child if I cannot be reached. I understand that any medical expenses incurred are my responsibility.
This authorization is limited to the medical treatment described in this section and is not a substitute for a medical power of attorney.
I agree to provide accurate emergency contact information and disclose any medical conditions, allergies, injuries, or other relevant concerns through the Parent Portal. I will notify HCB promptly of any changes to this information.

PHOTO & MEDIA RELEASE
I grant Hudson Conservatory of Ballet (HCB) permission to photograph, record, and use my child's image, likeness, voice, and performance in photographs, videos, digital media, printed materials, social media platforms, marketing materials, publications, and other promotional content created or distributed by HCB.
I understand that these materials may be used without compensation and may be edited, reproduced, published, or distributed by HCB for educational, promotional, or marketing purposes.
If I do not wish for my child's image to be used, I understand that I must notify HCB by email, text message, or by updating the photo release preference within my student account. This preference will remain in effect until changed by the account holder.
Declining this photo and media release will not affect your child's enrollment or participation in HCB programs.

COMMUNICATION & POLICY ACKNOWLEDGMENT
I understand that Hudson Conservatory of Ballet (HCB) communicates important information through email, text message, the Parent Portal, the studio website, social media, and other official communication channels.
I understand that it is my responsibility to review communications regularly and remain informed of studio policies, schedules, performance information, deadlines, dress code requirements, and other important updates.
I acknowledge that studio policies and procedures may be updated periodically to support the safety, operation, and educational goals of the studio. I understand that the most current policies will be available through the Parent Portal and/or official HCB communications.
Failure to read or receive communications does not exempt me from complying with studio policies, deadlines, financial obligations, or participation requirements.
By submitting this form electronically, I consent to sign this document electronically and acknowledge that my electronic signature is legally binding to the same extent as a handwritten signature.
