By signing this document, I acknowledge that I (or my child, if the participant is under 18) am voluntarily participating in dance classes and related activities at Pure Movement. I understand that there are inherent risks involved in physical activities, including but not limited to strains, sprains, and other injuries. I (or my child) accept full responsibility for any risks, injuries, or damages sustained as a result of participation, and I agree not to hold Pure Movement, its instructors, or staff liable for any claims arising from my (or my child’s) participation.

If the participant is under 18, I, as the legal guardian, am signing on their behalf and accept all terms and responsibilities outlined in this waiver.

I hereby authorize Pure Movement and its instructors to seek emergency medical treatment on my (or my child’s) behalf in the event of an accident, injury, or illness occurring during participation in any class, rehearsal, or performance. I understand that every effort will be made to contact the listed emergency contact before any medical action is taken. I assume full responsibility for any and all medical costs incurred.

If the participant is under 18, I, as the legal guardian, am signing on their behalf and assume responsibility for any medical decisions and costs.

I understand and agree to the following payment terms for classes with Pure Movement:

Tuition for group and private dance lessons must be paid in full before the start of each session or class.
Payments can be made via Zelle, Venmo, Credit Card, Cash or Check.
No refunds will be issued for missed classes or cancellations, except in cases of medical emergencies with a doctor’s note.
If I (or my child, if the participant is under 18) wish to cancel enrollment in any class or program, I must provide a minimum of 7 days’ written notice to receive credit toward future classes.
Failure to make timely payments may result in the suspension of class participation.
If the participant is under 18, I, as the legal guardian, accept the responsibility for making payments in accordance with this policy.

I, the undersigned, hereby acknowledge that participation in dance classes, rehearsals, and performances with Pure Movement involves physical activities that may result in injury. I voluntarily release Pure Movement, its instructors, and staff from any liability for injuries, loss, or damages incurred during my (or my child’s) participation in these activities. If the participant is under 18, I, as the legal guardian, am signing on their behalf and understand that I am responsible for the participant’s health and well-being during all classes and events. I agree to hold Pure Movement harmless for any claims arising from their participation.

I would like to receive emails from Pure Movement Inc, as it pertains to the classes we're enrolled in, any updates & upcoming events.
