I acknowledge that participation in dance and associated physical activities involves inherent risks, including the risk of injury.

By allowing my child to participate in classes, rehearsals, performances and any activities provided by The Dance Academy, I voluntarily assume all such risks on behalf of my child. I understand the importance of my child following instructions, safety guidelines and studio rules set by instructors and staff.

I agree to release and indemnify The Dance Academy, its directors, employees and contractors from any liability for injury, loss or damage arising from my child's participation, except where such liability arises due to negligence by The Dance Academy.

I understand dance is a hands-on art form and that teachers, and on occasion other students, may use appropriate physical contact to assist with alignment, technique and corrections.

I agree to adhere to all studio policies and guidelines outlined in the Parent and Student Handbook.

Fees and Tuition Policy

I understand and agree that:

Monthly tuition fees are all-inclusive and cover registration, class fees, administration, end-of-year costume hire, production fees, concert video, photo day, extra concert rehearsals and registration charges.

Annual tuition is calculated for the full year and divided into 11 equal monthly payments from February to December.

Monthly fees remain the same regardless of the number of lessons in a month or studio closures due to school or public holidays.

Students participating in concerts must be enrolled and have tuition paid for December

I give permission for The Dance Academy to use video footage and/or photographs of my child/children/self as students of The Dance Academy.

This usage may include (but is not limited to) displaying publicly, distributing, or publishing photographs and/or videos for use in materials such as:

printed materials (for example, brochures and newsletters)

online and offline advertising and promotional materials

videos and digital images, including use on social media platforms

I acknowledge that I am granting unrestricted permission for my child’s image to be used in print, video, and digital media. I understand that these images may be used by The Dance Academy for a variety of lawful promotional and educational purposes, and that this may occur without further notification or compensation.

I understand that no identifying personal information (including surname or location) will be used in conjunction with any images or videos.

If I do not wish to provide consent, I will notify The Dance Academy by email info@thedanceacademy.com.au

I acknowledge that, while The Dance Academy will make reasonable efforts to avoid using images or footage of students who have withdrawn consent, there remains a small risk that my child may appear incidentally in group photographs or videos taken during classes, performances or events that are published in the mediums above.
