SOMA Dance + Wellness — Parent Portal Agreement
By accessing and using the SOMA Portal, you acknowledge that you are 18 years of age or older and are the parent, legal guardian, or authorized caregiver of the registered dancer(s).
You agree to provide accurate and up-to-date information regarding your account, contact details, and dancer registration. You understand that this portal is used for communication, scheduling, billing, and class management related to SOMA Dance + Wellness programs.
You are responsible for monitoring your account activity and ensuring timely payments and class enrollment updates. Unauthorized use or misrepresentation of a dancer’s information may result in suspension of portal access.
By proceeding, you acknowledge that you have read and understood this agreement and consent to receive electronic communications from SOMA Dance + Wellness regarding classes, events, and account activity.
Privacy Notice:
SOMA Dance + Wellness respects your privacy. All personal and payment information submitted through this portal is securely stored and used solely for administrative and communication purposes within SOMA. Your information will never be sold, shared, or distributed to third parties without your consent.

SOMA Dance + Wellness — Minor Participation Waiver & Release of Liability
By enrolling a dancer in classes or programs at SOMA Dance + Wellness, I acknowledge and agree to the following:
I am the parent, legal guardian, or authorized caregiver of the registered dancer. I understand that participation in dance, movement, Pilates, and related activities involves physical activity and carries inherent risks, including but not limited to falls, sprains, strains, or other injuries. I voluntarily assume all such risks on behalf of the dancer.
I release and hold harmless SOMA Dance + Wellness, its owners, instructors, staff, volunteers, and affiliates from any and all liability, claims, or demands for injuries, damages, or losses arising from participation in any class, rehearsal, event, or activity on or off premises, whether caused by negligence or otherwise.
In the event of a medical emergency, I authorize SOMA Dance + Wellness staff to seek necessary medical care for my dancer if I cannot be reached and release them from any liability arising from such care.
I understand that classes, events, or rehearsals may be photographed or recorded for educational or promotional purposes. I grant permission for my dancer’s likeness to be used in SOMA materials unless I provide written notice otherwise.
For the health and safety of all participants, I agree not to bring my dancer to class if they are exhibiting signs of illness, including but not limited to fever, cough, sore throat, vomiting, diarrhea, or contagious infection. I understand that SOMA Dance + Wellness reserves the right to send a dancer home if symptoms of illness are observed.
By completing registration, I confirm that I have read, understood, and agree to the terms of this waiver and release.
