MDA WAIVER

Payment Policy: Tutition is due by the 1st. of every month. If payment is not received 5 days after the due date a $20.00 late fee will be added to your account. If tuition is not received 10 days after the due date an additional $10.00 will be added to your account. This also applies to any additional cost with set due dates as to when they are to be paid.

Cancel Policy - If the studio has to close due to bad weather or cancel a class, the student will be able to make up that class throughout the year in any other class or genre' within their age group.

I understand that dance and or Gymnastics (ACRO) is a physical and athletic activity and that participation may involve risk of injury, including but not limited to strains, sprains, or falls. I voluntarily assume all risks associated with participation and agree to release Milford Dance Academy, its staff, and instructors from liability for injuries that may occur during dance or gymnastic activities. I confirm that the participant or (participants) is physically able to take part in dance or gymnastics activities.

I, the undersigned parent / legal guardian, give permission for my child to participate in dance, tumbling, gymnastics, and related activities offered by the Milford Dance Academy. I understand that participation in these activities involves inherent risks and possible injury. I voluntarily assume these risks on behalf of my child.

I certify that my child is physically able to participate. I agree to release and hold harmless the studio, the owner, instructors, employees, and volunteers from any claims or liability arising from my child's participation.

By accepting / signing below, I aknowledge that I have read, understood, and voluntarily agree to the terms of this waiver.
