LIABILITY: I do hereby release and forever discharge Kiddos and Kin LLC, Shanynn Ryan and any affiliate associated with Kiddos and Kin and the owners of any facilities where classes are held from any and all actions, claims, and demands for, upon, or by reason of damage, loss, or personal injury which may be sustained by my child or myself during the course of, or as a result of, this musical activity. I hereby agree to inform any adult who accompanies my child to class of this release of liability.
