Illuminate Dance Project reserves the right to terminate lessons to any student without notice. In such a case a refund for unused lessons will be given.

I understand that IDP reserves the right to cancel a class for a season if enrollment is below five students.

I understand that IDP reserves the right to combine classes with enrollment that is less than 5 students.

I understand that IDP is not responsible for lost, stolen, or unclaimed items at the studio.

I give permission to IDP to include my child in photographs, video recordings, printed material and/or local media stories about IDP. I understand IDP will use these materials for promotional use.

I understand that as a parent or guardian, I should never interrupt a class to bring up a concern. All concerns should be brought to Georgia directly, under no circumstance should a concern be brought to an instructor without Georgia's knowledge.

Privacy policy: All customer information is private. Information will not be shared with third parties for direct marketing purposes.

Student Behavior. IDP is committed to providing a safe and supportive environment for all students. Students are expected to conduct themselves in a respectful and safe manner at all times. Students are expected to follow all IDP rules, instructions from staff, and safety guidelines. Students are to refrain from engaging in any behavior that could harm themselves or others.

Parents and guardians agree to ensure their child(ren) understands and complies with studio rules and expectations and discuss the importance of safety with their child(ren). Parents will inform IDP of any medical conditions or special needs that may affect their child(ren)’s participation. To the extent allowed by law, parents agree to indemnify and hold harmless IDP and its staff from any claims, damages, or liabilities arising from a student’s conduct.

Any incidents involving harm or potential harm must be reported to staff immediately. IDP will investigate reported incidents and take appropriate action, including but not limited to suspension or expulsion of students whose behavior does not comply with IDP policies and expectations.


I, the parent or legal guardian of my child, hereby give permission for my child to participate in all activities with Illuminate Dance Project (IDP). I agree that we will comply with IDP’s rules, policies, and procedures as they exist from time to time. I understand that dance instruction performance, and related activities involve some risk of injury or harm, and I assume all liability (whether for injuries, illness, or otherwise) arising out of my child's participation in any such activities. I agree to indemnify and hold IDP, its teachers, dancers, staff members, owners and employees harmless participation in any Dance Studio activity. I understand that IDP is a licensed, accredited, and insured organization. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our activities. I/we agree to report the unsafe conduct or conditions to the Director, instructor or staff member as soon as possible. Furthermore, if Illuminate Dance Project determines that an injury suffered by my child requires medical attention, I understand that IDP will make a reasonable effort to contact me. I hereby give permission for emergency medical treatment to be given to my child by medical providers chosen by IDP, however, in the event that IDP representatives are unable to reach me. By signing below, I further bind myself, my child, and our respective heirs, executors, administrators, successors, and assigns.
