I, the undersigned, allow my child to participate in all dance activities offered. I understand that these activities involve physical exertion.
I acknowledge and agree to the following:
● I am voluntarily participating in the dance activities at my own risk. I am aware of the physical demands and potential risks associated with these activities, including the risk of injury, illness, or death.
● I am in good physical condition and have no medical conditions that would prevent me from participating in the activities.
● I understand that Dent Dance Center and its staff are not responsible for any injuries, damages, or losses that may occur as a result of my participation in the Activities, including those caused by the negligence of its personnel.
● I hereby release and discharge Dent Dance Center, its owner, and its staff from any and all claims, liabilities, demands, actions, or causes of action arising out of or related to my participation in any activity.
● I agree to indemnify and hold harmless Dent Dance Center, its owner, and its staff from any claims or liabilities arising from my actions or behavior during the activities.
● I agree to follow all Dent Dance Center rules, guidelines, and instructions provided by staff during my participation in the activities.
I HAVE CAREFULLY READ AND FULLY UNDERSTAND THIS WAIVER AND RELEASE FORM. I AM AWARE THAT BY SIGNING THIS FORM, I AM WAIVING CERTAIN LEGAL RIGHTS, INCLUDING THE RIGHT TO SUE FOR ANY INJURIES OR LOSSES I MAY INCUR. I VOLUNTARILY AGREE TO ALL TERMS AND CONDITIONS STATED ABOVE.
