I, the parent of the child being enrolled, fully understand that the staff members of Aries Gymnastics LLC are not physicians or medical practitioners of any kind. With that in mind, I hereby release the staff to render first aid to my child in the event of injury or illness, and if deemed necessary by the staff to call our doctor and see if medical help, including transportation by a staff member of Aries or its representatives, whether paid of volunteer, to any health care facility or hospital, or the calling of an ambulance for said child should the staff deem this to be necessary.

The staff of Aries recognizes its obligation to make students and their parents aware of the risks and hazards associated with the sport of gymnastics, trampoline, tumbling, cheerleading and dance. Gymnastics, trampoline, cheerleading and dance can be dangerous and can lead to injury. Students may suffer injuries, possibly minor, serious, or catastrophic in nature. Parents should make their children aware of the possibility of injury and encourage their children to follow all the safety rules and coaches instructions. With the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child or children participate in the programs offered by Aries Gymnastics and Cheer. I, do hereby fully and forever release, discharge, indemnify and agree to hold harmless Aries, its agents, servants and employees from any and all claims, demands, damages, rights of action of causes of action, present or future, whether the same be known, anticipated or unanticipated, resulting from or arising out of participation in gymnastics, trampoline, tumbling, cheerleading, or dance instruction, or open workouts or in the case of any exhibition, competition, or clinic in which child may participate while traveling to or from the event.

I also affirm that I now have and will continue to provide proper hospitalization, health, and accident insurance coverage which I consider adequate for both my child's protection and my own protection.

I also understand that it is the parents’ responsibility to warn the child about the dangers of gymnastics and injury. The parent should warn the child according to what the parent feels is appropriate. Aries Gymnastics and Cheer will only warn the child through “safety messages” and our teaching style and progressions.

Compliance with Rules: I agree to comply with all rules, regulations, and instructions provided by Aries Gymnastics Facility staff members during gymnastics or cheer activities. My child and I will act with respect and care to themselves and all other participants in the program.

Parental Consent (if participant is under 18 years old): As the parent or legal guardian of the participant named above, I hereby consent to their participation in gymnastics or cheer activities at Aries Gymnastics Facility. By typing my name and date in the section below I confirm and acknowledge that I have read and understand the terms of this release waiver.

Students may be photographed or videoed during class for promotional purposes.
