Your child must be 3 years of age by December 31st to enroll in class.
I/we realize that participation in dance and/or yoga classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By filling out the registration form, I/we assume all risks related to the use of any and all spaces used by Creative Kids Movement Network (CKMN). I/we realize that CKMN is not responsible for missing dance shoes, yoga mats or other personal belongings. I/we authorize CKMN to use photos and videos in advertising. I/we agree to the Payment Information listed on the CKMN website, including the class break schedule, missed classes, and weather cancellation policy. I/we understand that tuition payment cannot fall more than one month behind in order for my/our child to attend class. Dance recital acknowledgement (school year dance classes only): I/we understand there is an additional recital fee of no more than $150 (including costume) and agree to pay in two equal installments in November and February.
If your child is withdrawn from class mid-month during full-season classes, tuition for that month is non-refundable. If your child is withdrawn from class during a Session, a portion (up to half) of the tuition may be refunded.
If your child discontinues their class before its scheduled end date, you must email your child's Program Director to drop them from the class, and tuition must be paid to date. Please note that if they are a dance student and it is after recital fees have been paid, the fees are non-refundable.
