You must be over 18 to register for an account. By creating an account, you acknowledge that you are the legal guardian of the student(s) being registered and agree to follow all studio policies, terms, and conditions.

In consideration for participating in dance classes, rehearsals, workshops, performances, and all other activities offered by Danzone (the “Studio”), I, the undersigned, acknowledge and agree to the following terms:

Assumption of Risk

I understand that participation in dance activities involves inherent risks, including but not limited to slips, falls, sprains, strains, fractures, and other injuries, up to and including the remote possibility of death.

I willingly assume full responsibility for any injury, illness, loss, or damage that may result from my or my child’s participation in Studio activities, on or off the premises, or while traveling to and from events.

Release & Waiver of Liability

I voluntarily release, discharge, and agree not to hold Danzone, its owners, directors, instructors, employees, or agents liable for any injury, illness, disability, loss, or death resulting from participation, including claims caused by ordinary negligence.

Medical Authorization

In the event of a medical emergency, I authorize Danzone to seek medical attention for my child from licensed medical providers.

I understand I am solely responsible for all medical expenses incurred.

I certify that my child is in good health and fit to participate.

Rules & Policies

I agree to follow all Studio rules, regulations, and policies.

I accept responsibility for any damages caused by myself or my child.

Photo & Video Release

I give permission for Danzone to use photos and/or videos of my child and/or family for promotional, advertising, and social media purposes.

Payment & Cancellation Policy

All tuition and fees are non-refundable.

No credits, refunds, or transfers will be issued for dropped, missed, or canceled classes (including in-studio, virtual, outdoor, mobile, and private lessons).

Enrollment cancellations must be submitted at least 2 weeks in advance.

Severability

If any part of this agreement is found invalid, the remainder shall remain in full effect.

Acknowledgment

I have read, fully understand, and voluntarily agree to this release of liability.

I am the parent/guardian of the minor listed below and am signing of my own free will.
