1. ACKNOWLEDGMENT AND ASSUMPTION OF RISK
I acknowledge and understand that participation in dance, tumbling, and related physical activities at Blue Bird Studio LLC involves inherent risks, including but not limited to falls, sprains, broken bones, and other injuries that may result from engaging in physical activity or the use of studio equipment or facilities. I voluntarily assume all such risks, known and unknown, on behalf of myself or my child.

2. WAIVER AND RELEASE OF LIABILITY
In consideration of being permitted to participate in classes, events, and activities offered by Blue Bird Studio LLC:

I, the undersigned, hereby release, waive, and discharge Blue Bird Studio LLC, its owners, officers, instructors, agents, employees, volunteers, and affiliated entities from any and all claims, demands, causes of action, liabilities, costs, and expenses (including attorney fees) arising from any injury, loss, or damage to person or property that may occur while participating in any activity, including travel to and from the studio, whether caused by negligence or otherwise.

This waiver and release applies to myself as an adult participant, or on behalf of my minor child.

3. MEDICAL CONSENT
I certify that I/my child is physically able to participate in all activities and classes. In the event of an emergency, I authorize Blue Bird Studio to seek medical treatment for myself or my child, and I agree to be financially responsible for any resulting medical expenses.

4. AGREEMENT TO FOLLOW RULES
I agree to abide by all rules and regulations set forth by Blue Bird Studio LLC and understand that failure to do so may result in suspension or termination of participation without refund.

5. SEVERABILITY
If any portion of this agreement is held to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

I HAVE READ THIS RELEASE OF LIABILITY, FULLY UNDERSTAND ITS TERMS, AND AGREE TO IT FREELY AND VOLUNTARILY.

CONSENT FOR PHOTO & VIDEO USE

I hereby grant Blue Bird Studio, LLC the absolute and irrevocable right and permission to:

Photograph and/or record my child or myself during classes, performances, and events.

Use, reproduce, publish, distribute, and display these images or videos in any and all forms of media, including but not limited to the studio’s website, social media platforms, advertisements, and printed materials.

Edit, alter, or use in combination with other media without restriction.

I understand that:

I will not receive compensation for the use of these photos or videos.

All media becomes the property of Blue Bird Studio, LLC and will not be returned.

I waive any right to inspect or approve the finished product.

Tuition Payment Terms

Tuition for each class is due on the first day of each month.

A $5 late fee will be added if payment is not received by the seventh day of the month.

Late Fee & Class Suspension

If tuition (plus the $5 late fee) remains unpaid more than one week late—that is, by the seventh day of the month—the student will be placed on hold and removed from class until the account is brought current.

To be reinstated, the full overdue amount plus the late fee must be paid.

Acknowledgment of Responsibility
I, the undersigned parent or guardian, understand and agree to the following terms:

Tuition is due monthly on the 1st.

A $5 late fee will be assessed if payment is not received by the 7th day.

If tuition and late fees are not paid by this deadline, my child will be removed from class, with re-enrollment contingent upon payment of all outstanding amounts.

I am responsible for staying current on my account to ensure uninterrupted class participation.
